PartⅠPurpose:The purpose of this study was to analysis the risk factors of complications in patients who required interventional therapy for peripheral arterial disease.Assess the complications risk before operation,and reduce complication during perioperative period.Material and method:Data from 302 patients collected with a retrospective review were statistically analyzed, during December 2005 and December 2010. Collection messages include:base massage, history, symptoms, signs, examinations, and operation messages. To consider thrombogenesis, angiorrhexis, haematoma, pseudoaneurysm, arterio-venous fistula as local complication.Cases was divided into local complication group and control group. Local complication group include 16 cases(male 12 cases; female 4cases, age 71.2±8.4), control group include 286 cases (male 190 cases; female 96cases, age 66.7±13.1).Figure out risk factors by comparing data between tow groups.Result:1. 66.7% cases in local complication group had smoking history, higher than control group, which is 37.7%, showed significant difference after statistical analysis (P<0.05)2. Diastolic blood pressure of local complication group were 76.3±9.5mmHg,lower than control group,which was 82.5±14.5mmHg,showed significant difference after statistical analysis (P<0.05)3.The rate of calcification in local complication group is higher than control group, showed significant difference after statistical analysis (P<0.05)3.According the classification, type 4 lesions lead to higher local complication rate, showed significant difference after statistical analysis (P<0.05)4. According to DSA result, type 1 and type 3 lesion have lower complication, and type 4 lesion have high local complication.showed significant difference after statistical analysis (P<0.05)5. Take count of smoking, diastolic blood pressure, calcification and the typing of each case, and make a ROC Curve.Aera is 74.4%.When the cutoff value is 4, Sensitivity is 27.3%, Specificity is 100%.Conclusion:Risk factor of local complicate incidence of interventional therapy for peripheral arterial disease.1.Smoking increase the local complicate incidence of interventional therapy for peripheral arterial disease.2. The relationship between blood pressure and local complicate incidence is close, the increase pulse pressure will increase the incidence of local complication.3. Calcification in superficial femoral artery is an indicate that tend to local complication.4. According to DSA result, type 1 and type 3 lesion have lower complication, and type 4 lesion have high local complication.5. System complication group include 11 cases (male 6 cases; female 5 cases, age 76.6±11.9), control group include 291 cases (male 196 cases; female 95cases, age 66.6±12.9).Figure out risk factors by comparing data between tow groups.Result:Risk factors for system complications.1. Age of system complication group were 76.6±11.9,higher than system group,which was 66.6±12.9,showed significant difference after statistical analysis (P<0.05)2. 45.5% cases in system complication group had cardiac disease history, higher than control group, which is 14.4%, showed significant difference after statistical analysis (P<0.05)3. 63.6% cases in system complication group were typed as Fontaine IV, higher than control group, which is 16.7%, showed significant difference after statistical analysis (P<0.05)4. System complication group have higher WBC, PT, APTT, and lower Hb,showed significant difference after statistical analysis (P<0.05).but not clinical significance.5. System complication group have higher X-ray positive,showed significant difference after statistical analysis (P<0.05)6.The rate of complication between System complication group and control group is difference in different type of lesion, showed significant difference after statistical analysis (P<0.05)7.Take count of age, cardiac disease history, Fontaine IV, X-ray positive and the typing of each case, and make a ROC Curve.Aera is 74.4%.When the cutoff value is 4, Sensitivity is 27.3%, Specificity is 100%.Conclusion:.Risk factor of system complicate incidence of interventional therapy for peripheral arterial disease.1. Pneumonia, COPD, cardiac enlargement, which can be identify by X-ray have a higher risk to system complicate incidence.5. X-ray positive means more system complication.6. The rate of complication between System complication group and control group is difference in different type of lesion, type 4 lesion have high local complication.7.
