Objective:To evaluate the cognition degree of knowledge about disease in patients with chronic obstructive desease(COPD) in Kunming at the preset.Methods:1. Survey methods:All residents 40 years of age or older were randomly selected to interview a face to face,with a standardized questionnaire. Spirometry was performed on all eligible participants.The questionnaire includes generar (age,gender,height,weight, education, income,ect),awareness of the disease,awareness of the AECOPD, cognitive the therapy of COPD.2. Statistical menthod:questionnaire data were coded and entered into Microsoft EpiDate 3.02 by two persons independently, with SPSS 17.0 version statistical package for statistical analysis.Basic descriptive analysis,count data using the Chi-square test, measurement date using analysis of variance,T-test rank sum test, ranked date using rank sum test,multivariate analysis using multiple liner regression analysis.Results:Among 2503sampling subjects,2193 participants completed the questionnaire and spirometry (response rate 87.6%). The overall prevalence of COPD was 4.2%.The avenage age is 70.7±7.56 years.97.8%of the patients had medical insurance. COPD prevalence of 1.0 to 20.0 years in age,with an average of 3.5 years. According to the《2007 diagnosis and treatment of chronic obstructive pulmonary disease guidelines standards)), in 92 COPD patients the prevalence of stage I (mild), stage II (moderate), stage III (severe), and stage IV (very severe) COPD wasl0(10.9%),41(44.6%),35(38.0%), and 6(6.5%).Seventy two patients (78.3%) did not attend any form of the activities of COPD.As shown in our study,about thirty four patients (40.0%) with COPD including 26 (61.2%)male and 8(32.0%) female,have home oxygen therapy, there are two patients are asymptomatic. There are 46 cases had not been diagnosed COPDo Never did pulmonary function test is 46 cases(50.0%),35 male (52.2%),11 female(44.0%).Male smoking rate was significantly higher than female (X2=32.888, P<0.001), second-hand smoking was no significant difference between male and female.Of all patients with COPD,43 patiens had never heard COPD.When we asked "which the following major diseases were related with COPD?",the first answer is "do not know",the second is "chronic bronchitis or emphysema".the rate is 54.3%,42.4%.When asked "which of the following factors are with COPD? ",the most frequent answer is "do not know",48 case(52.2%), the followed is "smoking and second-hand smoking".when asked "what do you think the symptom of COPD",the first answer is short breath,the proportion is 47.8%,the scend is "do not know",the rate is 42.4%. When asked "what are the main symptoms of AECOPD",the first answer is "syspnea" (42.4%),the second is "cough" (33.7%). Other chronic diseases not related to the prevalence of COPD, the highest prevalence of hypertension, accounting for 46.7%, followed by coronary heart disease,accounting for 7.3%. Quality of life in COPD patients was positively correlated with economic conditions,the better the economic situations the higher quality of life in COPD patients. Classification and severity negatively correlated, that is the more severe COPD disease, lower quality of live.Conclusion:paientis with COPD in Kunming have sericous lake of COPD knowledge.There are about half of patients do not know what is COPD and COPD with which factors.50.0%of patients were diagnose with COPD.50.0%never had pulmonary function tests in patients with COPD.It is time to strengthen the community health education,while COPD threament is not standardized.Disease impact on daily life greatly. We need to strenge management of pations.
