The SCD40L Level Of The Patients With Unstable Angina And Metabolic Syndrom And Its Relation With The Severity Of Coronary Artery Lesions

Objective:Coronary heart disease is significantly associated with metabolic syndrome,but the mechanism is still unclear.Focusing on unstable angina pectoris,the present study is to investigate the relationship between plasma sCD40L and the degree of coronary artery stenosis,and to find the impact of metabolic syndrome on coronary atherosclerosis.Methods:Patients enrolled between july 2010 and November 2011 all have clear diagnosis, including 30 patients with unstable angina,30 patients with unstable angina and metabolic syndrome and other 30 for control. The plasma sCD40L is measured by Enzyme-linked immunosorbent adsorption test,the degree of artery stenosis was confirmed by coronary angiographies and assessed quantitatively with Gensini score.Result:(1)The plasma sCD40L levels of the UA group were significantly higher than that of control group (4074±161 vs 1639±94 ng/ml, P<0.05), The plasma hs-CRP levels of the UA group were significantly higher than that of control group (30.9±1.8vs 11.9±0.6 mg/L, P<0.05), The Gensini score of the UA group was significantly higher than that of control group(56.6±3.3vs1.6±0.5, P<0.01); The plasma sCD40L levels of the UA and MS group were significantly higher than that of control group (5507±170vs4074±16 ng/ml, P<0.05), The plasma hs-CRP levels of the UA and MS group were significantly higher than that of control group (43.6±1.8vs11.9±0.6 mg/L, P<0.05), The Gensini score of the UA and MS group was significantly higher than that of control group(75.8±3.4vs1.6±0.5, P<0.01); The plasma sCD40L levels of the UA and MS group were significantly higher than that of U A group (5507±170vs4074±16ng/ml, P<0.05), The plasma hs-CRP levels of the UA and MS group were significantly higher than that of UA group (43.6±1.8vs30.9±1.8 mg/L, P<0.01), The Gensini score of the UA and MS group was significantly higher than that of UA group (75.8±3.4vs56.6±3.3, P<0.01);(2)In the control group,the plasma hs-CRP levels of male were higher than that of female of the same age level(P<0.05); the plasma hs-CRP levels of female more than 50 years old were higher than that of female less than 50 years old(P<0.05);In UA and MS group, the plasma sCD40L levels of the patients more than 50 years old were significantly higher than that of patients less than 50 years old of the same sex(P<0.05); the plasma sCD40L levels of the patients with higher SBP (SBP>130mmhg) were significantly higher than that of patients with lower SBP (SBP<130mmhg) in every group(P<0.05);similar result had been found with the plasma hs-CRP levels except UA and MS group.(3)In UA group and UA and MS group, the plasma sCD40L level was found to be positively correlated with FBS(r=0.365,0.101, P<0.05),negatively correlated with HDL-C(r=-0.079,-0.091, P<0.05); the plasma hs-CRP level was found to be positively correlated with FBS(r=0.315 P<0.05,r=0.651 P<0.01) and WC(r=0.086.0.231, P<0.05),negatively correlated with HDL-C(r=-0.061,-0.134, P<0.05).(4)In UA group and UA and MS group, the plasma sCD40L level was found to be positively correlated with Gensini score (r=0.678,0.929 P<0.05)and the plasma hs-CRP level(r=0,903,0.9 P<0.05).Conclusion:(1)Compared with the control group,the UA groupand the UA and MS group has higher level of plasma sCD40L and hs-CRP,suggesting a clear inflammation state with both groups.(2) The plasma sCD40L level is significantly correlated with the degree of coronary artery stenosis,so it can be seemed as an important indicator of the degree of coronary artery stenosis.(3)The plasma sCD40L level is correlated with hs-CRP,and they can interact each other inducing the develop of AS.(4) The plasma sCD40L and hs-CRP level and Gensini score are higher in UA and MS group than that of UA group,suggesting that MS can aggravate AS through inflammation by sCD40L...
