Purpose:To investigate the status of deciduous caries and early childhood caries among children aged from 2 to 4 years old in Changsha; Try to find the major risk factors of caries in this age group, so as to provide epidemiological guideline for the prevention of dental caries.Method:Adopt random cluster sampling among 2,435 children aged from 2 to 4 in 22 kindergartens of Changsha. By following the method which had been used in the Third National Oral Health Investigation, we carefully examed the oral health of all the children. Meanwhile, a questionnaire survey was submitted to all the parents. The questionnaire is divided into five major elements: 1. general situation,2. bom statues,3. eating habits,4. oral hygiene,5. family factors.Three groups was divided due to the result of the survey: 41 children with early childhood caries(dmft≥2),70 children with deciduous caries without ECC(dmft≠0)and 147 caries-flee children(dmft=0). All the results was analysis by using the SPSS 15.0 software, the t test, one way ANOVA, X2 test and logistic regression analysis statistical method was performed. Results:①The prevalence of deciduous caries of all samples is 33.71%, the mean dmft is 0.96,the mean dmfs is 1.59, and the filling rate is 12.04%. The prevalence of dental caries, mean dmft and mean dmfs are increasing gradually along with the children growing old. There are statistical significance in the distribution of prevalence rate of deciduous caries, mean dmft and mean dmfs in age(P<0.05). V tooth is the most likely tooth which can have a caries, while occlusal surface is the highest place where the early childhood caries can occur.②The prevalence of early childhood caries of all samples is 10.77%, the mean dmft is 0.44, The mean dmfs is 0.88, and the filling rate is 11.9%. The prevalence of early childhood caries, mean dmfs and mean dmft are increasing gradually as the children growing up. There are statistical significance in the distribution of prevalence rate of early childhood caries, mean dmfs and mean dmft in age(P<0.05). There are statistical differences in the rate of early childhood caries and mean dmfs in terms of the sex. Mesial surface is the highest in the early childhood caries rate, followed by lip surface.A close relationship was noticed between early childhood caries and caries in the primary molars(P< 0.05).③The result of logistic regression analysis shows that the variables Correlative Factors of Early Child Caries include maternal condition during pregnancy, gestation time, breastfeeding, brushing teeth before sleep, daily brushing frequency, the time when starting brushing, rinsing the mouth with fluoride water, maternal education, knowledge level of oral health, the number of breastfeeding, feeding chew food, and daily snaking frequency. Sleeping with pacifier may be the most important affecting reason for early childhood caries from within the age group of 2 to 4 years old.Conclusion:①The deciduous dental caries(33.71%) and early childhood caries(10.77%) of 2 to 4-year-old children have been reduced in comparison with previous year's survey of 2007. However, there still exist some problems such as early incidence of caries and the low filling rate.②Sleeping with pacifier is the most important affecting reason for early childhood caries within the age group of 2 to 4 years old, the protective factors of early childhood caries include: mental and physical health of mother during pregnancy, term pregnancy, breast feeding, tooth brushing before sleep, tooth brushing for several times a day, fluoride rinse, and the education level of parents.
