Objective: To investigate the recent effect about interventional treatment of complex lesions in arteriosclerosis obliterans on lower extremity.Methods: Clinical data of 35 patients (47limbs) who received interventional treatment between June 2009 and December 2010,those patients in TASC classification are all TASC C,D. To investigate whether it can get the satisfied effect and patency by using the interventional treatment and whether it is necessary for PTAS in the first operation.Result: 33 patients (45 limbs) had finished interventional treatment and had 55 stents placement. The length of the vascular lesions which covered by the stents is 13.2±5.5 centimeter. Those include: iliac artery 8 limbs, femoral-popliteal artery 26 limbs, both 11 limbs. In perioperative period, no patients were died.8 patients got complications and were all cured. The ABI of the remaining 31 patients was 0.65±0.33. On the basis of the effect standard, effectiveness, improvement, effectiveness are 25 limbs, 14 limbs, 6 limbs. The effect rate is 86.6%.The patients who received operations all have a higher ABI than preoperative confirmed by t test. The vascular patency rate of stent placement in iliac artery,femoral-popliteal artery,iliac and femoral-popliteal artery is 87.5%,88.5%,81.8%.The result of Fisher test shows those three group are no different.Follow-up:2 patients (2 limbs) were the lost of follow-up. The rate of follow-up is 93.9% and follow-up time is 3 to 20 months, 12 months for average. 3 patients who were recrudescence received operations and got better. Recent patency rate was:iliac artery 87.5%,femoral-popliteal artery 84%,the both 80%,the total was 83.7% .Conclusion: For all the patients of the TASC C,D classification, it is a good choice to get interventional treatment and implantation stents in the first operation whose main lesions is in iliac artery. For those patients whose main lesions is in femoral-popliteal artery and below, the effect and recent patency of interventional treatment and implantation stents in the first operation also a reasonable choice.
