| Objectives:To study the changes of plasma level of B-type Natriuretic Peptide(BNP)and serum level of high sensitivity C-reactive protein (hs-CRP) in the patients with coronary heart disease(CHD) only and coronary heart disease with three different types of glucometabolic abnormality as follows:Impaired Fasting Glucose (IFG),Impaired Glucose Tolerance(IGT),Diabetes Mellitus(DM). And the correlation of the severity of coronary artery lesions was analyzed by coronary angiography.Methods:According to the diagnostic criteria of coronary heart disease and glucometabolic abnormality,180 patients were divides as six groups:simple CHD,simple glucometabolic abnormality,CHD with IFG,CHD with IGT,CHD with DM,and normal controlled group (no coronary heart disease and no glucometabolic abnormality).Each group consists of 30 cases.Serum level of hs-CRP was detected with immune scattering than turbidity method and plasma level of BNP was detected with microparticle enzyme immunoassay.Results:(1) The serum level of hs-CRP and plasma level of BNP in simple CHD,simple glucometabolic abnormality group,CHD with IFG group,CHD with IGT group,CHD with DM group were higher than those in normal controlled group (P<0.05). The serum level of hs-CRP and plasma level of BNP in CHD with IFG group,CHD with IGT group,CHD with DM group were higher than those in simple coronary heart disease group and simple glucometabolic abnormality group (P<0.05).The serum level of hs-CRP and plasma level of BNP in CHD with DM group were higher than those in CHD with IFG group,CHD withIGT group,simple coronary heart disease group,simple glucometabolic abnormality group(P<0.05).The serum level of hs-CRP and plasma level of BNP in CHD with IFG group and CHD with IGT group were not statistically significant.(2) Single coronary vascular lesions in simple coronary heart disease show highest rate while single coronary vascular lesions in CHD with IGT show lowest rate.Double vascular lesions in CHD with IGT show highest rate while double vascular lesions in CHD with DM show lowest rate.Three vascular lesions in CHD with DM show highest rate while three vascular lesions in simple coronary heart disease show lowest rate (P< 0.05). Gensini integral quantity in CHD with DM group was higher than that in simple coronary heart disease group,CHD with IFG group,CHD with IGT Group (P<0.05). But the Gensini integral quantity mean still can not be considered as different in simple coronary heart disease group,CHD with IFG group and CHD with IGT Group (P>0.05).(3) The serum level of hs-CRP and plasma level of BNP in simple coronary heart disease group,CHD with different type of glucometabolic abnormality Group (IFG,IGT and DM) were positively correlated with Gensini integral quantity (r=0.919,P<0.05;r=0.906,P<0.05).hs-CRP and BNP level were positively correlated (r=0.830,P< 0.05). Multiple linear regression showns: hs-CRP and BNP were Gensini integral quantity level of independent forecast factor, hs-CRP were greater than BNP on Gensini integral quantity level role (β=0.542 andβ=0.476). Conclusions:(1) The serum level of hs-CRP and plasma level of BNP significantly increased in CHD with three different types of glucometabolic abnormality(IFG,IGT,DM) patients.Especially,hs-CRP and BNP increased in CHD with DM most significantly.(2) Two or more coronary vascular lesions rate increased in CHD with three different types glucometabolic abnormality(IFG,IGT,DM) patients.Especially,three coronary vascular lesions in CHD with DM show highest rate and most serious of pathological changes.(3) The level of hs-CRP and BNP were positively correlated with the severity of coronary artery lesions. hs-CRP and BNP were the independent forecast factor of severity of coronary artery lesions in CHD and CHD with glucometabolic abnormality. |
