Objective:to approach the relationship between the white matter lessions(WML)in aged people and mild cognitive impairment (MCI), and whether the white matter lessions is a risk factor of mild cognitive impairment or not. Method:50 clinical white matter lessions patients were made into research group, and 60 clinical non-white matter lessions subjects were made into control group according with MCI diagnostic and excluding standards of Petersen. All the subjects were evaluated with cognitive function, which included Mini-Mental State Examination, Montreal Cognitive Assessment, Clinical Memory Scale, Center of epidemiological survey depression scale and Global Deterioration Scale and Activity of Daily Living. The authors finished patient's blood pressure, then blood routine, liver and kidney function. All subjects tested MRI and were made into normal group mild WML group middle WML group and severe WML group.Group testing is made to all data, as well as analysis of covariance, x2 test, lineal correlative and logistic regressive analysis, so as to define relationship between the indices of the observation and MCI.Results:1.MCI was frequently detected in subjects with WML than in subjects without 2.WML was more frequently detected in MCI subjects than WML.in non-MCI (90%:43.3%, P<0.01),and the MCl subjects had higher prevalence of severe WML than control (34.0%:10.0%, P<0.05). 3.Scores of MMSE,MoCA were lower in MCI group than in non-MCI.. The scores of Neuropsychological examination decreased gradually as the increased with the severity of WML, it was statistically significant (P<0.05).4.The function of visuospatial/executive,naming,memory,attention,language,abstraction,orientation were reduced as the increased severity of WML,it was statistically significant (P<0.05).5.In correlation analysis among individual components of WML and each cognitives cores, the classification of WML was negatively associated with scores in MMSE,MoCA and several parts of MoCA(P<0.05) except the function of abstraction (P>0.05).6. In logistic regressive analysis, the last to enter the equation was the degree of education, ages, severe WML. Among these, high degree of education was the protective factor of MCI, ages, severe WML were the independent dangerous factors of MCI.. Conclusions:1. The patients' cognitive function will be aggravated when their WML achieve the certain degree. The degree of WML was negatively associated with the scores of Neuropsychological examination.2.The result indicating that the WML imposes impact upon various aspects of the cognitive function.3. Ages, severe WML are the independent dangerous factors of MCI, while high degree of education is the protective factor of MCI.
