Objective:Cardiovascular and cerebrovascular diseases have become the first killer for people's health, which mainly caused by atherosclerosis. Early diagnosis and intervention of atherosclerosis are the foundamental measures for prevention and treatment of the disease. This study is aimed at probing the risk factors for cardiovascular events in community-dwelling elderly, the predictive function of baPWV and its association with the carotid atherosclerosis, to provide preventive basis for cardiovascular and cerebrovascular disease in the elderly.Materials and methods:In 2005,725 retired military officers and their spouses,aged 60 or above,were selected to answer a questionnaire and receive Doppler echocardiogrphic examination for carotid intima-media thinkness(IMT) and common carotid artery(CCA),external carotid artety(ECA) and intenal carotid artety(ICA). Aortic elasticity was assessed by brachial-ankle pulse wave velocity(PWV) which was measured by an automatic device. Most of cases were investigated and received the above examinations again in 2010. Cardiovascular events in community-dwelling elderly and the association between the baPWV and carotid atherosclerosis are probed by multiplicity logistic regression analysis.Results:1. The investigation found that the prevalence of all causes death and cardiovascular events is 19.31%, hypertention 5 years ago, smoking and faster right baPWV are its independent predictive factors. Hypertention with faster right baPWV has the highest proportion in all causes death and cardiovascular events. The numbers of PCI and CABG increased in the people with right baPWV>1700cm/s.2. The study of PWV suggested that faster right baPWV is associated with age, drinking alcohol, history of hypertension, IMT of right common carotid artery, blood pressure and ABI of left and right limbs. Right baPWV is well associated with atherosclerosis in the same side.3. The positive rate for detection of plaque in carotid artery is 41.5% in this study, which is obviously increased compared with it 5 years ago. Longitudinal study for the relationship of baPWV and carotid artery atherosclerosis shows people who are male, have history of smoking 5 years ago, and right baPWV>1700cm/s have higher positive rate for detection of carotid artery atherosclerosis. Multiplicity logistic regression analysis shows that age and smoking are predictive factors for carotid artery atherosclerosis.Conclusion:Faster right baPWV may become predictive factors for all causes death and cardiovascular events independent of all others risk factors for cardiovascular disease. Faster right baPWV is influenced by age, drinking, history of hypertension and blood pressure. Right baPWV is well associated with atherosclerosis in the same side. Age and smoking are predictive factors for carotid artery atherosclerosis.
