Part1 Real-time shear wave elastography and ultrafast pulse wave velocity evaluate the nomal carotid arteryObjective:Normal common carotid artery wall was quantitatively analyzed by real-time shear wave elastography and ultrafast pulse wave velocity,and the hardness range of longitudinal and circumferential vascular wall of normal carotid artery was obtained.Methods:Between February 2016 and December 2016,120 cases of normal blood vessels diagnosed in physical examination center of the fourth affiliated hospital of Kunming Medical University were selected,aged from 20 to 70 years old,mean(48.94 ± 13.32)years.Among them,58 cases were male(49.33 ± 13.28)years old,62 cases were female,the average(48.58 ± 13.46)years old.Subjects were divided into 4 groups according to their ages:N1 group:32 people<40 years old average(30.81±5.57)years old;N2 group:28 people 41-50 years old average(45.79±2.01)years old;N3 group:30 people 51-60 years old average(55.07±2.35)years old;N4 group:30 people 61-70 years old average(65.10±2.12)years old.Using the French Supersonic Imagine's Aix-plorer color Doppler ultrasound diagnostic apparatus.Start shear wave elastic imaging mode,the mean value,the maximum value,the minimum value,standard deviation of the anterior wall and posterior wall of the common carotid artery were measured.Start ultrafast pulse wave velocity mode,PWV in the beginning of the systol and PWV in the end of the systol and the standard deviation of PWVBS and PWVES were measured.Results:1.Real-time shear wave elastographyThere were no significant differences in the mean value,max value,minimum value and dispersion of the anterior wall and posterior wall in the two sides of the normal carotid artery(P>0.05).There were no significant differences in the mean value,maximum value,minimum value and standard deviation of the anterior wall and the posterior wall of the common carotid artery in male group and female group.(P>0.05).There was a significant difference in the mean value,max value,minimum value of the elastic modulus between the anterior wall and the posterior wall of the normal carotid artery(P<0.05).The total mean value,max value,minimum value of the elastic modulus between groups of the anterior wall and the posterior wall of the common carotid artery had significant difference(P<0.05).The mean elastic modulus of the anterior wall of the common carotid artery was positively correlated with age(r = 0.476,P<0.01).The minimum elastic modulus of the anterior wall of the common carotid artery was positively correlated with age(r =0.554,P<0.01).The maximal elastic modulus of the anterior wall of the common carotid artery was positively correlated with age(r = 0.512,P<0.01).The mean elastic modulus of the posterior wall of the common carotid artery was positively correlated with age(r = 0.502,P<0.01).The minimum elastic modulus of the posterior wall of the common carotid artery was positively correlated with age(r = 0.498,P<0.01).The maximal elastic modulus of the posterior wall of the common carotid artery was positively correlated with age(r = 0.498,P<0.01).2.Ultrafast pulse wave velocityThere was no significant difference in pulse wave velocity betweent he begi-nning of the systolic period and the end of the systolic period of the two sides of normal common carotid artery(P>0.05).During the beginning of the systolic and the end of the systolic,there were no significant difference in pulse wave velocity between the male and female groups(P>0.05).The total pulse wave velocity in the beginning of the systolic and the end of the systolic between groups of the common carotid artery had significant difference(P<0.05).The pulse wave velocity in the beginning of the systolic of the common carotid artery was positively correlated with age(r = 0.349,P<0.01).The pulse wave velocity in the end of the systolic of the common carotid artery was positively correlated with age(r = 0.593,P<0.01).Conclusion:The hardness of the anterior wall of the common carotid artery were higher than that of the posterior wall under real-time shear wave elastic imaging.Two kinds of ultrasound imaging conditions:the common carotid artery wall hardness and age have a certain correlation,60-70-year-old common carotid artery wall stiffness was significantly higher than the age of 40 years of blood vessel wall hardness.with age,Vascular wall stiffness showed an increasing trend,41-50 years old and 51-60 years old carotid artery wall wall hardness has no difference.Common carotid artery wall hardness has nothing to do with gender.The hardness range of longitudinal and circumferential vascular wall of normal carotid artery are consistent.Part 2 Real-time shear wave elastography and ultrafast pulse wave velocity evaluate the hypertensive carotid arteryObjective:Common carotid artery wall of hypertensive patients was quantatively analyzed by real-time shear wave elastography and ultrafast pulse wave velocity,exploring the difference of the hardness of common carotid artery wall in different grades of hypertension,comparing with the normal group,and then evaluate the value of these two ultrasound imaging techniques in the common carotid artery wall of patients with hypertension.Methods:113 patients with hypertension diagnosed in our hospital were selected,aged 35-70 years,mean(52.54±13.41)years,including 60 males and 53 females.1 hypertensive group 38 cases,2 hypertensive group 38 cases,3 hypertensive group 37 cases.The first part of the 120 cases of normal carotid artery were selected as the normal group.Start shear wave elastic imaging mode,the mean value,the maximum value,the minimum value,standard deviation of the anterior wall and posterior wall of the common carotid artery were measured.Start ultrafast pulse wave velocity mode,PWV in the beginning of the systol and PWV in the end of the systol and the standard deviation of PWVBS and PWVES were wall of common carotid artery:There were significant differences in the mean,max and minimum values of elastic modulus between HBP3 and HBP2 groups(P<0.05).There were significant differences in the mean,max and minimum values of elastic modulus between HBP2 and HBP1 groups(P<0.05).There were significant differences in the mean,max and minimum values of elastic modulus between HBP3 and HBP1 groups(P<0.05).Hypertension group of the posterior wall of common carotid artery:There were significant differences in the mean,max and minimum values of elastic modulus between HBP3 and HBP2 groups(P<0.05).There were significant differences in the mean,max and minimum values of elastic modulus between HBP2 and HBP1 groups(P<0.05).There were significant differences in the mean,max and minimum values of elastic modulus between HBP3 and HBP1 groups(P<0.05).2.Ultrafast pulse wave velocityThere was no significant difference in pulse wave velocity betweent he begi-nning of the systolic period and the end of the systolic period of the two sides of common carotid artery of hypertensive patients(P<0.05).There was significant difference between the normal group,HBP1 group,HBP2 group and HBP3 group in pulse wave velocity in the beginning of the systolic and the end of the systolic of common carotid artery of hypertensive patients(P<0.05).During the beginning of the systolic and the end of the systolic,there were significant differences in pulse wave velocity between HBP3 and HBP2 groups(P<0.05).During the beginning of the systolic and the end of the systolic,there were significant differences in pulse wave velocity between HBP2 and HBP1 groups(P<0.05).During the beginning of the systolic and the end of the systolic,there were significant differences in pulse wave velocity between HBP3 and HBP1 groups(P<0.05).Conclusion:The hardness of the anterior wall of the hypertensive common carotid artery were higher than that of the posterior wall under real-time shear wave elastic imaging.Two kinds of ultrasound imaging conditions:With the increase in hypertension series,the thickness of vascular wall showed an increasing trend.The hardness of the hypertensive common carotid artery wall was higher than that of the normal carotid artery wall.The hardness range of longitudinal and circumferential vascular wall of hypertensive carotid artery are consistent.
