Osteoarthritis is one of the very common diseases of old man.It is the frequenest reason of cue to restraint the daily activities of the patients and severity influence quality of life.The arthroplasty, remodeling of the joint,which relieve to the pain of patient and improve the joint range of motion.The patients request Totle Knee Arthroplasty(TKA),who different age, remodeling of the joint and the articular malformation.Although the technique of Totle Knee Arthroplasty is ripening, but during in the operation the patient still occur many complications. Risks associated with total knee arthroplasty are very dangerous(ie, fever infection, fluid overload, wound disunion, embolism, deep vein thrombosis,and loosening of prosthesis). Patients undergoing total knee arthroplasty are at high risk for postoperative anemia and allogeneic blood transfusions. Autologous blood transfusion is the collection of blood from a single patient and retransfusion back to the same patient when required. This is in contrast to allogenic blood transfusion where blood from Unrelated/anonymous donors is transfused to the recipient. The primary driving forces for the use of autologous blood transfusion are to reduce the risk of transmission of infection and to protect an increasingly scarce resource.This study to determine the clinical efficacy of a postoperative drainage autotrans-fusion system.Object:Re-Transfusion in the postoperative of Total Knee ArthroplastyMaterials and methods:This study records of 66 TKAs that had been performed with a primary cemented, posterior stabilized TKA by the senior author between November 2008 and May 2010 at the First hospital of Ji Lin university were retrospectively reviewed. Knee prostheses were manufactured by Stryker (Stryker Corp, Kalamazoo, Michigan) or Zimmer (Zimmer Holdings Inc, Warsaw, Indiana). This study compared postoperative autotransfusi on drainage system and the practice of using allogeneic blood alone in patients undergoing TKA.The experimental group (postoperative Re-Transfusion): male:3cases, female:33 cases;age;46-79,average age:62.5. The control group (received common vacuum drainage system):male:5 cases,female:35 cases; age; 50-77,average age:61.5. Osteoarthritis(OA):62cases (93.9%), Rheumatoid arthritis(RA):4cases(6.1%). The hospital stay of the experimental group is 15.1 days. The hospital stay of the control group is 19.7 days. This study compared the RBC,Hb,and HCT in the 72h of postoperative,which with postoperative autotransfusion drainage system, and the practice of using allogeneic blood alone in patients undergoing TKA.Results:There were no statistical differences between The experimental group, and The control group of RBC,Hb,and HCT levels. Postoperative Re-Transfusion reduced the number of patients receiving allogeneic blood transfusion in TKA.In the postoperative, the volume of the experimental group loss blood is (641.46±283)ml,the volume of Re-Transfuse blood is (481.94±189.99)ml.Except some patients received allogeneic blood transfusion in bilateral TKA.The other patients didn't receive allogeneic blood transfu-sion.The volume of the control group loss blood is (658.62±310.68)ml, the volume of allogeneic blood transfusion is (3.21±1.89)u. The experimental group didn't occur the complications associated with blood transfusion.Conclusion:Autologous blood transfusion, when used appropriately, can provide a safe alternative to allogenic blood transfusion. There is an important need to balance use of resources in ensuring safe blood transfusion for all those who need it.However,there will always be not a redress anaemia.Despite evidence of efficacy, safety and cost-effecttiveness. However as allogenic blood becomes a scarcer resource the cost issue may become less important. Most emphasis should be on reducing blood transfusion generally.lt is likely that autologous transfusion may be increasingly used as part of a comprehend-sive blood transfusion strategy.
