In recent years, the incidence of ischemic heart disease has increased year by year,the hazards to human health is becoming more and more obvious.A lot of in vivo and in vitro experiments has been done both at home and abroad to improve treatment of myocardial infarction and reperfusion injury and verified some available drugs.It have great significance in guiding clinical treatment and improving patient outcomes. After acute phase, chronic myocardial ischemia can cause myocardial fibrosis, ventricular remodeling, so seeking effective drugs for treatment ischemic heart disease is our goal. Berberine is a kind of alkaloid extracted from plants, people in the past seem it as a antimicrobial in treatmenting infectious diseases.In recent years, we found that it has a wide range of pharmacological effects,such as anti-arrhythmic,anti-heart failure, anti-myocardial ischemia.Therefore berberine may play an important role in prevention and treatment of cardiovascular disease in future.Objective: To survey the protective effects of berberine on acute ischemia-reperfusion injury and ventricular remodeling after myocardial infarction, provide an effective treatment guidance for drugs clinical application in myocardial ischemia-reperfusion injury and myocardial disease.Methods: Through replicating the model of myocardial ischemia-reperfusion injury model in vivo,to record the changes of hemodynamic parameters and left ventricular function, the types and incidence and duration of ventricular arrhythmia, the sizes of myocardial infarct.It confirmed berberine have the effect of prevention and treatment in myocardial ischemia-reperfusion injury;Through the establishment of rat model of old myocardial infarction in vivo,to determine left ventricular function,heart coefficient and myocardial fibrosis,further more confirm that berberine can reverse left ventricular remodeling after myocardial infarction.Results:1.Effects of berberine on hemodynamics in acute myocardial ischemia-reperfusion rat:After the modeling,LVSP,LVDP,±dp/dt were significantly decrease after ischemia,then recovered after reperfusion,but were significantly lower than the level before ischemia in both Model and Ber groups; Compared with Model group , LVEDP were significantly lower (P<0.05) and LVDP were recovered after ischemia-reperfusion after oral administration in advance for 7days in Berberine group. (P<0.05).2.Effects of berberine on ventricular arrhythmia in acute myocardial ischemia-reperfusion rat:After ligation left anterior desending coronary artery in rats, only a small number of arrhythmia occurs in the Sham group,high incidence of lethal long-time ventricular tachycardia and ventricular fibrillation occurs in the Model group, Berberine group can decrease the incidence and the duration of ventricular tachycardia and ventricular fibrillation compared with the Model group (P<0.05).3.Effects of Berberine on myocardial infarct size in acute myocardial ischemia-reperfusion injury rats:After the experiment, AAR/LV% in Berberine and Model group was no significant difference between eath other. IS/LV% and IS/AAR% in Ber group were lower than Model group(P<0.05 , P<0.01).Compared with Model group, infarct area(white) significantly reduced in Ber group followed with Evans blue and TTC staining(P<0.05 , P<0.01).4. Effects of Berberine on systolic function of rats old myocardial infarct in vivo: Compared with model group ,berberine treatment group could significantly improve heart function, LVPWS, IVSs, LVFS and LVEF increased significantly in berberine treatment group, espically LVFS, LVEF (P <0.05).5. Effects of Berberine on myocardial fibrosis of rats old myocardial infarct in vivo: Comparison with model group, the heart of coefficient of berberine group decreased (p <0.05). The heart of coefficient have no significant difference among the control group ,the sham group and the berberine group ; compared with the berberine group, myocardial fibrosis (white) was obvious in the model group.Berberine treatment group can significantly decrease the degree of myocardial fibrosis and reduce the inflammatory infiltration.Conclusion: Effects of berberine on acute myocardial ischemia- reperf usion injury are obvious,firstly it can significantly improve left ventricular function after myocardial ischemia and reperfusion; moreover it can significantly reduce the incidence of reperfusion arrhythmia, reduce duration of reperfusion arrhythmia, reduce tolerance of ventricular tachycardia and ventricular fibrillation; further more significantly reduced myocardial infarct size after ischemia and reperfusion. Also effects of berberine on chronic myocardial fibrosis of myocardial infarction rats is obvious.It can significantly improved cardiac function of myocardial infarction rats; it can significantly reduced heart coefficient and the degree of myocardial fibrosis in myocardial infarction rats. All these proved that berberine may be a potential effective drug for ischemic cardiovascular disease.
