Objective: To discuss and analysis the aetiology and relevant prevention of early pain after total hip arthroplasty (THA).Methods: Retrospectively reviewed 210 cases (225 hips) treated with total hip arthroplasty including 195 (95.2%) unilateral THA cases and 15 (4.8%) bilateral ones of our department from January 2009 until January 2011, which were followed up at least 12 months (12-24 months,average 14 months), thereby analysis the aetiology, clinical feathures and prevention of early pain after THA.Results: Early pain were found in 24 cases (10.7%) including 7 cases(3.1%)with infection, 6 cases (2.7%) with loosening, 2 cases (0.9%) with aseptic inflammation, 3 cases (1.3%) with posterior dislocation, 2 cases (0.9%) with periprosthetic fracture, 2 cases (0.9%) with leg lenth discrepancy, 1 cases (0.7%) with lumbar spine stenosis and 2 cases with unexplained pain.Conclusion: (1) At clinical work, the causes of total hip arthroplasty are more complicated, especially diagnosis. Only on a comprehensive analysis of postoperative pain can we effectively avoid misdignosis. (2) By analyzing the causes of early postoperative pain, we found infection 3.1%, aseptic inflammation and loosening 3.5%, other reasons 4.4%. (3) For the 2 patients with unexplained pain, we considered incompletely synovectomy in sugery, witch is still needed to further prospective studies.
