Methods:We first selected 90 cases of the paraffin blocks filed by Department of Pathology, Affiliated Hospital of Luzhou Medical College from March 2007 to March 2010. These includes 20 cases of dysplasia (10 cases for moderate dysplasia and 10 cases for severe dysplasia) and 50 cases of advanced adenocarcinoma (15 cases for differentiated type,15 cases for well differentiated type and 20 cases for poorly differentiated type). At the meantime,20 cases of normal gastric tissue that are 5cm away from carcinoma tissue were chosen as control group. For each specimen, histological type was confirmed by pathological diagnosis. The expression of Survivin and Livin was detected in 10 cases of normal gastric tissues,20 cases of dysplasia and 50 cases of gastric carcinoma by immunohistochemistry S-P method. Using SPSS 10.0 for statistical analysis, count data were analyzed by chi-square test and Fisher exact probabilities test. Multi-factor Binary Logistic regression was used to analyze the correlation between Survivin and Livin with pathologic parameters, and correlation analysis was carried out by Spearman rank correlation analysis. Results:(1) The positive expression rates of Survivin in the normal gastric tissue, dysplasia and gastric carcinoma were 5%,30% and 74% respectively. The differences among them were significantly. The positive expression rates of Survivin in the gastric carcinoma was significantly higher than that in the dysplasia tissues (χ2=11.67, P<0.05) and normal gastric tissues (χ2=27.41, P<0.001). The positive expression rates of Survivin in the dysplasia was significantly higher than that in the normal gastric tissues (χ2=4.33, P< 0.05). The positive expression rates of Livin in the normal gastric tissue, dysplasia and gastric carcinoma were 5%,40% and 68% respectively. The differences among them were significantly. The positive expression rates of Livin in the gastric carcinoma was significantly higher than that in the dysplasia tissues (χ2=4.67, P<0.05) and normal gastric tissues (χ2=22.68, P<0.001). The positive expression rates of Livin in the dysplasia was significantly higher than that in the normal gastric tissues (χ2=7.03, P<0.05) (2) The expression of Survivin was closely related to the depth of invasion and lymph node metastasis of gastric carcinoma(P<0.05).There was no correlation among the expression of Survivin and gender and age of the patients, the grade of cell differentiation, and tumor size of gastric carcinoma (P>0.05); The expression of Livin was closely related to the lymph node metastasis of gastric carcinoma(P<0.05).There was no correlation between the expression of Livin and gender and age of the patients and the grade of cell differentiation, the depth of invasion and the tumor size of gastric carcinoma (P>0.05). (3)There wasn't significant correlation between Survivin and Livin in gastric carcinoma. (χ2=0.44, P>0.05) Conclusions:(1) The positive expression rates of Survivin in the gastric carcinoma was significantly higher than that in the dysplasia and normal gastric tissue, and the expression rates of Survivin in dysplasia is significantly higher than that in the normal gastric tissues. Besides that, the expression of Survivin is associated with the invasion depth and lymph node metastasis of gastric carcinoma, suggesting that the abnormal expression of Survivin may play an important role in the spread of the cancer, which makes it potentially useful in predicting the metastasis of gastric carcinoma. (2) The positive expression rates of Livin in the gastric carcinoma was significantly higher than that in the dysplasia and normal gastric tissue, and the expression rates of Livin in dysplasia is significantly higher than that in the normal gastric tissues. Besides that, the expression of Livin is associated with the lymph node metastasis of gastric carcinoma. Detection of Survivin expression can be used as an early indicator in diagnosis and prognosis of gastric carcinoma. (3)There wasn't significant correlation between Survivin and Livin in gastric carcinoma.
