Clinic Analysis For 27 Cases Of Paraquat Poisoning

Objective To summarize EICU paraquat poisoning patients some clinical features ,disccuss the experiences or lessons of blood purification treat for paraquat poisoning in the current and prognosis factors, provide the basis for clinical diagnosis and treatment.Methods The clinical data of 27 acute paraquat poisoning patients was collected during the time from January 1 to June 30,2010 in EICU of the first Affiliated Hospital of Guangxi Medical University, According to Statistics (research need to be designed) collected the basic situation of statistics all cases,clinical indicators and survival et al , then analysis these datas.Results The number of female was more than male among 27 acute paraquat poisoning cases ,High incidence groups were at the age of 20-40 ,the top organ which had highest incidence rate of organ damage was kidney (88.89%) ,and the rest organs were liver (62.96%), lung (51.85%) heart (33.33%) pancreas (14.81%), brain (3.7%), The case fatality rate of MODS with 1,2,3,4,5 or more organ injury death rates were 25%, 50%, 87.5%, 85.71%, 100%; The patients were divided into two groups by the number of organ damage (≤1 group ,≥2 group ) , Analysis showed that the survival rate demonstrated ststistical significance (P = 0.002); The percentage for the first blood purification is 14.81% within 8 hours and 40.74% after 24 hours, But only 15.79% were give HP within 8 hours, 63.16% for HP within 8-24 hours. Two cases are give CVVH treatment. Univariate analysis showed that poisoning volume, AST, ALT, TBIL, there are indications of whether the machine, WBC, APACHEⅡscore, length of stay, time of the first indicators of blood purification demonstrated ststistical significance (P <0.05) between death group and survival group. SIRS is the performance of the two groups (P = 0.023). Multivariate logistic regression analysis showed that APACHEⅡscore correlated with prognosis of acute paraquate poisoning .Conclusions EICU cases of acute paraquat poisoning need to prevent and cure SIRS or MODS .The opportunity and mode of blood purification choice is important, whether of affecting the prognosis still need further study. ICU patients treated early is still necessary, the Joint (HP + CVVH) treatment. Poisoning the amount , liver function, whether the machine indications, WBC, APACHEⅡscore, length of stay, the first time of blood purification are the assessment prognostic factors of paraquat. APACHEⅡscore can be used in patients with paraquat poisoning prognosis.
