OBJECTIVE: The nonsmall-cell lung cancer (NSCLC) patients with indications for radical surgery were treated with hyperbaric oxygen (HBO) before surgery. The Sa02, Pa02 and whole blood reduced viscosity of high, medium and low shear rates of the NSCLC patients were tested to study the effects of preoperative HBO therapy on improving patients' body tissue oxygen supply, decreasing blood viscosity, helping patients'recovery, reducing the time of postoperative stay and the incidence of postoperative complications aimed to figure out a new method to comprehensive treatment of NSCLC patients.METHODS: Forty-two NSCLC patients in the Fourth Affiliated Hospital of GuangXi Medical University between November 2009 and December 2010 were randomized into control group (n=21) and experimental group (n=21). The patients in the control group were treated with routine therapies before and after operation. The patients in the experimental group were treated with HBO(2 hours per day for 5 days with pressure of 0.2Mpa and surgery on the 6th day ) before surgery apart from routine therapies. The Sa02, Pa02, and whole blood reduced viscosity of high, medium and low shear rates were tested on the first day before hyperbaric oxygen therapy, on the first day before surgery, and on the third day after surgery. The postoperative hospital stay and complication rates of both groups were observed. The difference of all index at the corresponding time points, the postoperative hospital stay (significance level atα=0.05) and the rates of postoperative complications (significance level atα=0.05) were compared between two groups.RESULTS: (1) Before HBO treatment in the experimental group and before operation in the control group, there were no significant differences (P>0.05) in PaO2, SaO2, whole blood reduced viscosity (high, medium and low shear rates) between the two groups with comparability. (2) Since the HBO treatment was adopted in the experimental group, for the experimental group and control group, PaO2 of the former was significantly higher (t=2.993,P<0.05) than the latter. The former's whole blood reduced viscosities were significantly lower (tHigh=-8.592, PHigh<0.05; tMedium=-7.557, PMedium<0.05; tLow=-4.922, PLow<0.05) than the latter. But, there were no significant differences (t=1. 392, P>0.05) in SaO2 between two groups. (3) In experimental group,the PaO2 and SaO2 on the first day before surgery were significantly higher (P<0.05) than those index before HBO therapy. (4) In experimental group,the whole blood reduced viscosities on the first day before surgery were significantly lower (tHigh=6.628, PHigh<0.05; tMedium=6.398, PMedium<0.05; tLow=7.492, PLow<0.05) than the same index before HBO therapy. (5) On the third day after surgery, the PaO2 of experimental group was significantly higher (t=3.263, P<0.05) than that of the control group. For SaO2, it was similar to the above mentioned point (2). Besides, the experimental group's whole blood reduced viscosities were significantly lower (tHigh=-5.322, PHigh<0.05; tMedium=-4.695, PMedium<0.05; tLow= -3.191, PLow<0.05) than that of the control group. (6) The postoperative hospital stay in the experimental group ( average of 12.62±3.99d) was a little shorter than that of the control group ( average of 13.33±4.64d) without statistical difference (t=-0.535,P>0.05) and incidence of postoperative complications in the experimental group (14.3%)was a little lower than that of the control group (23.8%)without statistical difference(X2=0.154,P>0.05).CONCLUSIONS: HBO therapy before operation among NSCLC patients may obtain the following effects.①Lung function may be improved, the tolerance of patients against surgery may be improved and the risk of surgery may be reduced.②The blood viscosities may be significantly reduced which theoretically prevent or delay local recurrence and distant metastasis of cancer cell.③There's a trend that HBO therapy may shorten postoperative hospital stay and reduce the incidence of postoperative complications.
