Objective: To assess the effects of hyperbaric oxygen(HBO) combined with routine methods which were used to improve synthetically the placental microcirculation in the treatment of fetal intrauterine growth retardation(IUGR) by means of the Doppler ultrasound, electronic fetal monitoring, hemorheology and neonate outcome, also to compare the changes of hemorheology between two IUGR groups in different therapy ways, meanwhile the feasibility for HBO used in pregnancy will be discussed advancedly.Methods: 61 pregnant women were diagnosed as having fetal IUGR, who were demanded to be singleton pregnancy and have regular menstrual cycle and whose gestational age were between thirty-two weeks and thirty-eight weeks. There were no gestational infections and bad hobbies, excluding initially the cases of fetal malformation and pathological pegnancy.All cases were voluntarily assigned to two groups,a HBO group called group-A (n=28) receiving HBO combined with routine therapy and a non-HBO group called group-B(n=33) only receiving the routine therapy (salbutamol, dan-shen root, glucose, amino acids.).Both groups received treatment for five days. In addition, thirty cases with normal pregnant women were served as a normal control group called group-C(n=30). Ultrasound , non-stress test, umbilical artery blood flow velocity ,and hemorheology were performed periodically and compared among three groups .Results: 1 The comparison among the umbilical artery blood flow changes of three groups(l)Before treatment, the values of Umbilical artery pulse index (PI), resistant index (RI) and the rate of systolic flow velocity and diastolic flow velocity (S/D) of both group-A and group-B were significantly higher than those of group-C and the rate of the fast blood velocity( FBVR) wassignificantly lower than that of group-C before treatment.(2)After treatment, the values of S/D,PI and RI in group-A weresignificantly reduced than those of before treatment and the values ofFBVR was higher than that of before treatment.(3)The values of PI and RI in group-A were more significantly reduced thanthose of group-B after treatment.2 The comparison of changes among the hemorheology of three groups(1) The blood viscosity at the low shear rate(LBV) in group-A and group-c was significantly higher than that of group-C.(2) The value of LBV in group-A was significantly reduced after treatment and was significantly lower than that of group-B.(3) There was a significant negative correlation between the maternal LBV and the birth weight of their infants, and a positive correlation between the maternal S/D, PI and RI. There was a significant negative correlation between the maternal S/D, PI, RI and the birth weight of their infants .3 The abnormal non-stress test(NST) and S/D in group A showed a better recuperative tendency after treatment than that of in group-B. And there was a significant difference between them.4 The comparison of the fetal outcome(1) The delivery manner of group-A and group-B had no significant difference, but the rate of cesarean were significantly higher than that of in group-C.(2) The average gestational age in the different delivery manner had no significant difference among three groups. The neonatal average birth weight in group-A and group-B were significantly lower than that in group-C. But the average birth weight of neonate in group-A was significantly higher than that of in group-B.(3) The curative effect showed that the cure rate was 64.3% in group-A , higher than that in group-B(36.4%). There was a significant difference between them.-4-Conclusion: HBO can improve obviously the hemorheology of IUGR patients and the significant clinical effect can be obtained by means of HBO combined with routine method in the treatment of IUGR.
