[Objective] To determine serum adiponectin concentration in women with and without polycystic ovary syndrome (PCOS) and to assess possible correlations of adiponectin to the hormonal and metabolic parameters, including measures of insulin resistance (IR)[Methods] Forty women with PCOS and thirty-six infertile women without PCOS were included in the study. Serum levels of Follicle stimulating hormone (FSH), luteinizing hormone (LH), Testosterone (T), Prolactin (PRL), Estrodiol (E2) were measured. Serum levels of adiponectin, glucose and insulin were measured at Oh, 1h,2h during 75g oral glucose tolerance test (OGTT). To further investigate the relationship of adiponectin and insulin resistance, the women with PCOS were treated with Diane-35 and metformin 500 mg tid for 3 months. All indicatrix were measured after treatment. Measures of IR included Fating serum insulin (FINS), Homeostasis Model Assessment (HOMA-IR), insulin area under the curve (AUC ins) and Plasma glucose area under the curve (AUC pg). Height, weight, waist circumference and hip circumference were measured while the Body Mass Index (BMI) and Waist-to-hip ratio (WHR) were calculated.[Results]1,Women with PCOS (both lean and obese) showed significantly increased levels of LH, LH/FSH and T as compared with corresponding control groups (P<0.05). And levels of LH and LH/FSH differed significantly between lean and obese women with PCOS. While levels of FSH were found to be decreased in obese women with PCOS as compared with other groups. There was no difference in PRL and E2 in the four groups.2,FINS, INS 1h, INS 2h, AUC ins and HOMA-IR were higher in women with PCOS (P>0.05). While glucose levels were similar to controls (P>0.05), only AUC pg was found to be increased in obese women with PCOS as compared with nonobese women without PCOS.3,Adiponectin concentrations were found to be significantly decreased in women with PCOS and in obese women without PCOS as compared with lean women without PCOS.4,There was a significant increase in adiponectin levels when treated with metformin and Diane-35. While significantly decrease in FINS, INS 1h, INS 2h, AUC ins, HOMA-IR, LH, LH/FSH and T despite unchanged glucose, BMI, WHR and FSH.5,Pearson analysis showed that adiponectin concentrations correlated inversely with T, LH, LH/FSH, FINS, INS 1h, INS 2h, FPG, AUC ins, HOMA-IR and BMI but correlated positively with FSH. A stepwise multiple linear regression analysis showed that FINS, HOMA-IR enter the regression equation.[Conclusion]1,Adiponectin concentrations were found to be significantly decreased in women with PCOS and in obese women without PCOS as compared with lean women without PCOS.2,The PCOS patients treated with Diane-35 combined with metformin is one of better methods. It can not only improve insulin resistance (IR) but also increase adiponectin levels,3,We observed a strong correlation between serum adiponection and insulin resist-ance. Adiponectin will be a indicatrix for the diagnosis of IR of PCOS.
