Objective:To explore the function of Yishen compound capsule on TNF-a and AGEs in the occurrence and development of diabetic nephropathy (DN) and the mechanism of action and relationship between them.Methods:Forty eight male Wistar rats were divided into 6 groups. The normal control group (N), model group (M), Yishen compound capsule high dose group(T1), Yishen compound capsule medium dose group(T2), Yishen compound capsule low dose group(T3), and western medicine control group (X)( benazepril+ gliquidone). The Right kidney of 50 rats were surgically removed. After a week of wound healing, the rats were injected with 30mg ? kg-1 dose of streptozotocin (STZ) by intraperitoneal injection for 5 consecutive days to create the diabetic rat model [7] . Forty diabetic rat models were successfully created. A separate normal control group of 8 rats was created with intraperitoneal injections of an equal volume of citrate buffer. 8 weeks after the diabetic rat models were created, the body weight, fasting blood glucose, blood urea nitrogen, glycosylated hemoglobin, fasting insulin, and other indicators were measured. Fluorescence spectrophotometry was used to measure the relative amount of serum AGEs. left kidney biopsies were performed and renal histopathologic changes were observed with light microscopy. Immunohistochemistry SABC was also performed to measure kidney TNF-αexpression.Results:A comparison of the Yishen compound capsule groups and the model group after treatment showed that the Yishen compound capsule high and medium-dose groups'body weight, urine output, blood glucose and blood urea nitrogen levels significantly improved. At the end of 8 weeks, the high-dose group's BUN and HbAlc levels were lower (P<0.01) and lower serum insulin levels. TNF-αand serum AGEs expression in the renal tissue of the high-dose group were significantly lower than that of the model group (P<0.01). The high and medium Yishen compound capsule dose groups and western medicine control group showed inhibition of mesangial proliferation, basement membrane thickening, and a reduction in renal tissue TNF-αand serum AGEs expression, at the same time increasing insulin levels (P<0.01). This result was most pronounced in the western medicine control group , the high Yishen compound capsule dose groups result close to the western medicine control group (p>0.05).Conclusion:1. Yishen compound capsule can, to a certain extent, reduce BUN and HbAlc content in DN rats; Decreased serum insulin levels, and thus reduce insulin resistance in rats DN .It also reduces renal tissue TNF-αand serum AGEs expression.2. Kidney deficiency, blood stasis, and yin and yang deficiency is the main mode of pathogenesis of DN. Promoting blood circulation, dissolving stasis, and nourishing yin and yang is an important governance law for the treatment of DN using Chinese medicine. Its function of reducing the role of inflammation in DN may be one of its mechanisms of action.3. Yishen compound capsule has a protective effect on kidneys in diabetic rats.
