Objective: Through the inspection of OPN expression in normal cervical tissue, CIN, cervical cancer tissues in variable and different stages and different pathological type,The study analys the relationship between the OPN expression and cervival cancer of different pathological types,clinical stages,histologic differentiation degree,presence of lymph node metastasis. This can lay the foundation for OPN may become the tumor markers of the judgement of the cerical cancer prognosis and become target genes of cerical cancer treatment.Methods: 144 cases of specimens from June 2009 to December 2010 in the Third Clinical Medical College of Jilin University (China-Japan Friendship Hospital) and the Third Clinical Hospital of Jilin University of obstetrics and two out-patient clinics and hospitalized patients of cervical tissue wax pieces. Preoperative patients were not given any treatment. Divided into three groups: (1)the normal control group: benign uterine diseases without cervix, postoperative pathology confirmed 26 cases of normal cervix. Aged between 35-53 years old, average age 44.0±5.3years old; (2) CIN groups: in the parallel out-patient treatment of cervical cone excision, or because of cervical disease do the whole hysterectomy, postoperative pathology confirmed CIN,including 20 cases of CINⅠand 18cases of CINⅡ-Ⅲ, aged between 26-59 years old, average age 41.8±6.9years; (3) cervical cancer group: 80 cases of cervical cancer, aged between 28-69 years old, average age 46.5±9.3 years.According to the pathological type:67 cases of squamous cell carcinoma, 13 cases of adenocarcinoma, preoperative has cervical living tissue pathology. Reference to malignant tumor specimens according to FIGO staging in 2009, 59 cases ofⅠperiod, 21 cases ofⅡperiod, both the implementation of radical hysterectomy and pelvic lymph node dissection. According to the lymph node metastasis: 14 cases of lymph node metastasis; 66 cases of without lymph node metastasis. The pathological and histological grading: 17 patients less differentiation,42 cases of differentiation, 21 cases were well-differentiated.The average age of above three groups of comparison there was no significant difference(P<0.05), comparable. The clinical specimens organization obtained immediately bubble in 10% formalin fixed in liquid for 24 hours, conventional paraffin embedding, sectioning, made four microns slice; First HE dyeing, again slice stick add at coated with polyphosphate lysine slides in the reserve to do the next immunohistochemical stains, using immunohistochemistry S - P method (streptomycin resistance biotin protein a peroxidase).OPN positive expression is that in cell plasma and (or) on the membrane appear tan-yellow particles.The judgment standard according to the following two aspects: (1) by the proportion that positive cells in the total cells: 0 points, no positive fine cytoplasmic; 1 minute, positive cells < 25%, Two points, positive cells from 25% to 50%, Three points, positive cells > 50%.(2) by cells show color intensity : 0 points, cells show no color; 1 minute, the cells are yellow, Two points, cell is tan-yellow; Three points, the cells are brown.The positive is that (1)and(2)tested three points, otherwise negatie radiography. All slice both determinated by pathologists blinded.All datas are managed by SPSS16.0 software process . The correlation analysis of the counting material are usedχ2 inspection. P < 0.05 said difference was statistically significant.The results:①The OPN positive expression rate in normal cervical tissue is 3.84%; In CINⅠis 25%, In CINⅡ-Ⅲis 38.89%; In cervical cancer tissues is 67.5%. The OPN positive expression rate in CIN was significantly higher than in normal cervical tissue, in cervical cancer tissue was significantly higher than CIN. The OPN positive expression rate by normal cervical tissue - CIN - cerical cancer is increasing trend.The OPN positive expression rate in CINⅠis significantly higher than in normal cervical group, with significant differences between the two groups (χ2 = 4.460, P = 0.035 < 0.05), That in CINⅡ-Ⅲis higher than in CINⅠgroup, but two groups of comparisons was not statistically significant (χ2 = 0.846, P = > 0.05), that in cervical cancer is significantly higher than in CINⅡ-Ⅲgroup, with significant differences between the two groups (χ2 = 5.118, P = 0.024 < 0.05). Shows OPN in cervical cancer occurrence, development plays an important role.②80 cases of cerical cancer organization (according to the 2009 FIGO stage), includingⅠperiod 59 patients andⅡperiod 21 cases .Two groups statistical comparison, the results show that 36 cases ofⅠperiod are positive expression, expression rate is 61.02%, 18 cases ofⅡperiod are positive expression, expression rate for 85.71%.Ⅰperiod andⅡperiod compared :the positive expression rate ofⅡperiod was significantly higher thanⅠperiod , statistical analysis greup (χ2 = 4.306, P = 0.038 < 0.05). OPN positive expression rate is heighten with clinical stage of cerical cancer increasing , explain OPN positive expression related with the clinical stages of cerical cancer.③80 cases of cerical cancer organization , including 67 cases of cervical squamous cell carcinoma, 13 cases of cervical adenocarcinoma. the OPN positive rate of Cervical squamous cell carcinoma tissues is 65.67% (44/67), the OPN positive rate of Cervical adenocarcinoma is76.92% (10/13). There is no significant difference in two kinds of pathological type (χ2 = 0.628, P > 0.05), 0.428 (see chart 2). Explain OPN positive expression and pathological types of cerical cancer is irrelevant.④The cerical cancer can be divided into less differentiation group, a moderately differentiated group and well-differentiated groups according to the histopathological differentiation degree.The OPN positive expression rate of poorly differentiated group is 94.12% (16/17) ,that is 69.05% in a moderately differentiated group (29/42), The OPN positive expression rate of well-differentiated groups is 42.86% (9/21). The OPN positive expression rate gradually raised with cerical cancer histopathological differentiation degree reducing, and three groups of OPN positive expression rate compared greup. Explain the masculine expression and cervical cancer OPN histological differentiation degree are concerned.⑤Cerical cancer tissues, the OPN positive expression rate in lymph node metastasis and without lymph node metastasis respectively 92.86% (13/14), 62.12% (41/66). Two groups of comparisons OPN positive expression rate greup (χ2 = 4.974, P = 0.026 < 0.05). Explain OPN positive expression and cervical cancer of lymph node metastasis are concerned.Conclusion:1. The OPN positive expression rate is increasing trend by normal cervical tissue - CINⅠ- CINⅡ-Ⅲ-cerical cancer. The OPN positive expression rate in cancer tissue is significantly higher than that of normal and CIN group, showed OPN in cervical cancer occurrence, development plays an important role.2. Clinical stages of cerical cancer the later, organization differentiation degree lower, who had lymph node metastasis ,OPN positive expression rate of the higher. Explain the OPN positive expression and the clinical and pathologic stage organization differentiation degree and presence of lymph node metastasis relevant, with the development of cerical cancer, OPN expression increase. Through testing the OPN expression in cerical cancer tissues,it has the important meaning on the judgement of cerical cancer malignant degree, prognosis. Make OPN may become judgment cerical cancer prognosis of the tumor markers.3. the OPN positive expression rate in cervical tissue has nothing to do with cervical cancer pathological type.4. OPN is the gene expression product related to tumors metastasis, it can stimulate tumor invasion and metastasis through many ways.The in-depth study of OPN, make its can become gene therapy target genes, its in cervical cancer treatments have broad application prospect.
