ObjectiveTo study the changes of urine NGAL in patients of type 2 diabetic mellitus (T2DM) and the association of NGAL with MAU(microalbuminuria). To investigate the clinical role of NGAL in the pathogenesis of vascular endothelial dysfunction, insulin resistance.MethodsAccording to the standards of diabetic diagnosis and typing put forward by WHO in 1999, a total of 60 patients with T2DM were selected. Based on albumin creatinine ratio (UACR) of real-time urine samples, the patients with type 2 diabetes were divided into two groups:30 patients with normal albuminuria and 30 patients with microalbuminuria.30 healthy volunteers from conventional physical people were enrolled as control.. All subjects were measured NGAL, fasting blood glucose, fasting insulin, blood lipid, HbA1c, urine albumin urine creatinine and BMI, HOMA-IR were calculate. The data were ananlyzed by ststistical software SPSS 13.0 version.Results1. The urine NGAL level in patients of type 2 diabetes were higher than normal control group (44.11±3.51 vs 24.29±3.51, P<0.01)2. The urine NGAL level in patients of type 2 diabetic with microalbuminuria were significantly higher than patients with normal albuminuria (50.67±5.47 vs 37.54±4.16, P<0.05) and normal control (50.67±5.47 vs 24.29±3.51, P<0.01)3. A correlation analysis showed that there were remarkable possitive correlation between NGAL and UACR, HOMA-IR, HbA1c, FBG, TG,TC and no correlation between NGAL and BMI, WBC,LDL,HDL.4. Multiple stepwise regression analysis showed that NGAL and HOMA-IR remained the influence factors for UACR in type 2 diabetic patientsConclusionThe level of urine NGAL in patients of type 2 diabetes elevated, the level of urine NGAL in patients of type 2 diabetes with microalbuminuria increased significantly. Furththermore, NGAL positively correlated to HOMA-IR. NGAL may take part in the development of vascular endothelial dysfunction and insulin resistance in patients with T2DM.
