Research On The Problems Of Link-up Between The New Cooperated Medical System And The Medical Assistance In Rural Areas

For six years'pilot and perfect, the Medical Assistance in rural areas and the New Cooperated Medical System have achieved a certain success. These two systems are combined with each other in actual operation of most areas.As a important part of rural medical security system, the link-up between the two systems has greatly reduced the phenomenon that the peasants suffer poverty from disease, and improve the peasants'hospitalization environment. But as a complex system engineering, the link-up between the Medical Assistance in rural areas and the New Cooperated Medical System has met a lot of problems since it promoted. If we can't solve these problems appropriately, it must influenc the long-term development of the link-up between the two systems. The significance of this paper is to analysize and solve the problems of link-up between the Medical Assistance in rural areas and the New Cooperated Medical System, and with the purpose of furnishing scientific basis for the decision-making of the government as well as promoting the development and perfection of rural medical security system.By the combination of normative analysis and empirical analysis, comparative analysis, quantitative analysis and qualitative analysis, this paper has made a deep research on the problems of link-up between the New Cooperated Medical System and the Medical Assistance in rural areas, and the following conclusions are drawn.(1)The Medical Assistance in rural areas and the New Cooperated Medical System are integral parts of rural medical security system, which are also important act to reduce the phenomenon that the peasants suffer poverty from disease. There are both similarities and differences between the two sysyems, which just bring high significance to the link-up between the Medical Assistance in rural areas and the New Cooperated Medical System.(2)The link-up between the Medical Assistance in rural areas and the New Cooperated Medical System is necessary. Basic medical security could be provided for the impoverished population depending on the link-up between the two systems.( 3 ) There are typical modes of link-up between the Medical Assistance in rural areas and the New Cooperated Medical System, such as the Chongqing Mode, the Hubei Mode, the Hunan Mode, the Jiangsu Mode and the Sichuan All-in-One Mode. The similarity of these typical modes is regarding the Medical Assistance in rural areas as a an effective medium in which incomplete rural medical security system is linked up, and their difference lies in their developing level.(4)The problems of link-up between the Medical Assistance in rural areas and the New Cooperated Medical System are as follows, problems of system design, problems of operational mechanism, problems of regulatory mechanism and problems of information management. Problems of system design refers to complicated procedures for reimbursement, unstability in financing mechanism of the New Cooperated Medical System, low level medical assistance because of insufficiency of Medicaid fund. Problems of operational mechanism refers to insufficiency of administration fund and the deformity of management mechanism in the Medical Assistance system. Problems of regulatory mechanism refers to that the fund regulation is not in place and there is not systemic restraint designated health-care institutions. Problems of information management refers to incompleteness of information system in the Medical Assistance system. (5)Measures should be taken to enforce the link-up between the Medical Assistance in rural areas and the New Cooperated Medical System, which include standarding functioning system, establishing and perfecting operational mechanism, strengthening supervision system and tightening up the management. To be more specific, compensation mechanism of health service need to be reasonably designed. We can make the right changes for the medical assistance order, and regulate medical assistance procedure. Measure must be adopted to perfect compensation mechanism and create the management system and the evaluation mechanism with high efficiency. Supervision mechanism must be set up, and effective regulation and supervision should be adopted to medical fund and medical institutions . Information system in the Medical Assistance system should be built by actively coordinating with relevant national sectors as a mean to share information between the Medical Assistance in rural areas and the New Cooperated Medical System.(6)These principles must be followed to accelerate the link-up between the Medical Assistance in rural areas and the New Cooperated Medical System: the principle of meeting the demands of peasants'basic medical care; the principle of intercoordination; the principle of the leading position of the government; the principle of tailoring measures to suit local conditions step by step; the principle of being efficient and convenient; the principle of advancing sustainable development of the system.(7) Supplementary measures should be taken to promote the link-up between the Medical Assistance in rural areas and the New Cooperated Medical System. Specifically speaking, we should build rural health care network, and improve medical care service quality. The government should provide strong support and implement systematic planning, and speed up the economic development in order to create favourable conditions for the development of the sytems. Furthermore, the work of linking up with other systems should be well done.
