Gastric cancer is malignant tumor of digestive tract. Lymph node metastasis is the way of transference and is an important factor in prognosis. In recent years, with the discovery of VEGF-C, lymph node metastasis has attracted more and more attention. The newly discovered LYVE-1 and D2-40 were considered as lymphocyte endothelial-specific markers, which can detect the existence of lymphatic vessels. Much study has showed that the lymph node metastasis play an important role in the process of tumor metastasis, and the tumor incidence of lymphatic invasion is prerequisite for lymph node metastasis. By detecting the density of lymphatic vessels in tumor tissue, gastric cancer can explore lymph angiogenesis and lymph node metastasis, which can be further prediction of tumor lymph angiogenesis and to determine the incoming transfer, so as to provide a basis for clinical treatment.Objective:The purpose of this study was to explore the relationships between lymph angiogenesis and lymph node metastasis by investigation and comparison of the two new lymphatic-specific markers LYVE-1, D2-40, their expression and clinical significance of gastric cancer in normal tissues and different stages of different classification of gastric cancer tissues.Methods:1.50 gastric cancer samples were selected among years 2007.6 to 2007.12. Each specimen was diagnosed by pathology. 30 samples of chronic gastritis tissues were confirmed by electronic endoscopy biopsy and pathological specimens and each group of samples was adopted RT-PCR to detect the gene expression in LYVE-1. 2.Elivision immunohistochemistry was used to detect the expression of LYVE-1 and D2-40 in gastric cancer, and analysis of the relationships with clinic pathological features of gastric cancer associated.Results:1. Results from RT-PCR: The positive expression rate of LYVE-1 in 50 cases of gastric cancer was significantly higher than that in chronic gastritis tissues (P<0.05); low differentiation was obviously higher than well and moderate differentiation; distant metastasis was higher than those without distant ones in gastric cancer (P<0.05).2. Results from Immunohistochemistry2.1 The positive expression of LYVE-1 was defined as brown-yellow stain which located in the endothelial cell of lymphatic vessels. The LVD with LYVE-1-positive in gastric cancer was significantly higher than those in chronic gastritis tissues.(7.01±3.47 piece/HP vs1.22±1.41 piece/HP,P<0.05), while peritumoral tissues was markedly higher than that in intratumoral tissues (10.43±3.23 piece /HP vs4.13±1.55 piece /HP,P<0.05). The low differentiation was obviously higher than that in well and moderate ones while stageⅢ-Ⅳwas significantly higher than that in stageⅠ-Ⅱ(8.53±1.39 piece/HP vs4.28±2.43 piece/HP,P<0.05);and the LVD with LYVE-1-positive in lymphatic or distant metastasis was obviously higher than those without lymphatic(8.53±1.36 piece/HP vs5.38±2.46 piece/HP,P<0.05) or distant metastasis(9.55±1.21 piece/HP vs6.28±2.46 piece/HP,P<0.05)respectively.2.2 The expression of D2-40 in normal tissues and gastric cancer: The positive expression of D2-40 was defined as brown-yellow stain which located in the endothelial cell of lymphatic vessels. The LVD in gastric cancer(8.01±4.47 piece/HP) was significantly higher than that in chronic gastritis tissues(1.32±1.41 piece/HP) (P<0.05);the LVD in peritumoral tissues(11.49±3.53 piece/HP) was markedly higher than that in intratumoral ones(4.53±1.75 piece/HP) (P<0.05);the LVD with D2-40-positive in low differentiation was obviously higher than those in moderate and well ones(9.06±1.64 piece/HP vs6.15±2.95 piece/HP,P<0.05), and stageⅢ-Ⅳwas obviously higher than that of stageⅠ-Ⅱ(9.06±1.39 piece /HP vs4.28±2.43 piece/HP,P<0.05), while the LVD with D2-40-positive in lymphatic transference was obviously higher than those without lymphatic metastasis (9.10±1.36 piece/HP vs7.38±2.46 piece/HP, P<0.05),The LVD with D2-40-positive in distant metastasis was obviously higher than those without distant metastasis (10.55±1.21 piece/HP vs7.38±2.46 piece/HP,P<0.05).2.3 The comparison of the positive rate between D2-40 and LYVE-1: in the same condition, the positive rate of D2-40 was higher than LYVE-1 despite of the clinical stage, or degree of differentiations, or distant of lymphatic metastasis.2.4 The correlation between the positive expression of LYVE-1 and D2-40: the expression of LYVE-1 and D2-40 was positively correlated.Conclusion:1.The positive expression of LYVE-1,D2-40 was defined as brown-yellow stain which mainly located in the endothelial cell of lymphatic vessels.2.The relationship between the LVD in gastric cancer and the extent of differentiation of gastric cancer and with or without lymphatic or distant metastasis was corelated..3.the marking effect of D2-40 is better than that of LYVE-1.
