Influence Of Rhubarb And Licorice Decoction Enema Treatment Based On Syndrome Differentiation On The Severity Of Sepsis With Intestinal Dysfunction

Objective:To observe the clinical effects of Rhubarb and Licorice Decoction Enema treatment based on syndrome differentiation on Sepsis with intestinal dysfunction.And its influences on the severity of Sepsis and prognosis of patients are also assessed.Method:Using method of retrospective study, totally 136 example cases of ICU of Guang'anmen Hospital, dated from July 2005 to June 2009, were selected. And all the cases meet the diagnostic criteria of Sepsis with intestinal dysfunction.136 patients were divided into the Treatment Group with 55 patients and the Control Group with 81 patients. The Treatment Group were offered Rhubarb and Licorice Decoction Enema treatment (once per day) based upon routine treatment, while the Control Group received routine treatment only. Intestinal dysfunction score, APACHE II score, laboratory data, gut tolerance to enteral nutrition, before treatment and 7 days after treatment,14 days mortality and the average survival time of death patients in the ICU are selected as the main observing indexes for clinical effcts.Results:1. Syndrome Distribution in Chinese Medicine of the Treatment Group:the proportion of patients with stoppage of fu-qi only are 19 cases(34.5%), complicated with Deficiency of Qi arel2 cases (21.8%), complicated with yin-deficiency are 13 cases (23.6%), complicated with Phlegm are 7 cases (12.7%), complicated with yang-deficiency are 2 cases (3.6%), complicated with Blood-Stasis are 2 cases (3.6%).2. Rhubarb and Licorice Decoction Enema treatment based on syndrome differentiation have good better curative effect on intestinal dysfunction. Treatment Group were improved on Intestinal dysfunction score compared with Control Group (P<0.01). Both groups were improved on gut tolerance to enteral nutrition, and treated group is better than control group (P<0.05).3. Rhubarb and Licorice Decoction Enema treatment based on syndrome differentiation can remarkably improved severity and prognosis of sepsis. Compared with before treatment, APACHE II score descend obviously in the Treatment Group(P<0.05) while there was no significant difference in the Control Group. And lower 14 days mortality are observed in Treatment Group, compared with that of Control Group(P<0.01). And survival time of death patients in the ICU in Treatment Group is longer than that in Control Group(P<0.01).4. Rhubarb and Licorice Decoction Enema treatment based on syndrome differentiation can reduces inflammation. In Treatment Group, WBC count and CRP level after treatment are both decreased(P<0.05).5. Coagulation index is not considered as the main evaluation index for clinical effects. Coagulation indexes are in the normal range in both groups before and after treatment, with no significant difference.Conclusions:Rhubarb and Licorice Decoction Enema treatment based on syndrome differentiation can improve Sepsis with intestinal dysfunction and gut tolerance to enteral nutrition, reduce the severity of Sepsis, decrease the mortality of patients and improve the prognosis of Sepsis.
