Abstract/Summary:
[Background ] Cervical cancer is the most common gynecological malignancies in the world.As we all know that Human papillomavirus (HPV) persistent infection with high-risk (HR) HPV is recognized as a necessary cause of cervical cancer. So, not only in screening cervical cancer, but also in guiding the treatment of cervical cancer, considering the prognosis of cervical cancer, choosing appropriate methods of treatment of cervical cancer after treatment follow-up to monitor the possibility of recurrence, HPV testing has played a very important effect. However, the cervical cancer incidence and prevalence data is not complete, school-age women with HPV infection is unknown, distribution of HPV high-risk genotypes is unknown, incomplete epidemiological data makes the decline in cervical cancer prevention and control efforts. Therefore, there is an urgent need for such data to develop prevention programs.So far, HPV still can not be cultured in the organization cells and is not easy to use serology for epidemiological investigation, and reproductive tract infections often do not have the performance of serology. Therefore, direct to the reproductive tract epithelial cells, HPV DNA testing has drawn increasing attention. HC-Ⅱis the most method of detection of HPV in worldwide, not only can detect high and low risk HPV types, but also to determine the levels of viral load, to offer some clinical significance. Chinese territory is wide, different regions, different ethnic groups, or even in different occupations in different living environments the type of women infected with HPV is not the same, Different types in different regions due to the severity of cervical lesions,so understanding distribution of an area infected with HPV high-risk type , it can guide the clinical work in this region and to offer clinical data for preventive vaccine.[Objective]1) To investigate the HPV DNA load and to explore in uterine cervix with different pathological changes.2) To investigate distribution of HPV genetypes in Hefei and to explore the relationships between multiple infections and different cervical lesions[Methods]1) HCⅡwas used to detect HPV DNA load in different cervical lesions.2) To detect distribution of HPV genotypes in different cervical lesions by Flow-Through Hybridization And Gene Chip(HybriMax).[Results]1) Detection of HPV DNA load by HCⅡ, CINⅠ~Ⅲand cervical cancer compared with the control group, there was significant difference (P <0.05); and there was no significantly difference between CINⅠ~Ⅲand cervical cancer (p>0.05).2) In Hefei ,the most common high risk HPV types were HPV16(50.0%)and 52(26.7%),followed by 33(11.4%),58(8.5%)and 18(6.4%);Low risk types 11(8.5%) and 6 (3.4%)were prevalence;Among Chinese popular types HPV 66 was higher prevalence.3) Single and multiple infection rate was 62.7% ( 148/236 ) and 37.3%(88/236),respectively.Dual infection was main type of multiple infection,the most common models were HPV16/52 combined with other types.CINⅡ~Ⅲmultiple infection rate was higher compared to negative or cervix,CINⅠand cervical cance(rP<0.05).Sigle infection was more common in cervical cancer. [Conclusion]HPV DNA load levels does not mean that the severity of the disease, however, the higher HPV DNA load the greater risk of CIN and cervical cancer. HPV16,52,18,33and 58 are the most prevalence high-risk subtypes,in Hefei.Double infection especially HPV16/52 in conjunction with other subtypes are associated with CINⅡ~Ⅲ,but not cervical cancer. Detection of HPV DNA load and genotype is likely to be to improve early diagnosis of cervical cancer and provide more effective and more reasonable program, and may be as a monitoring and evaluating indicators of cervical disease prognosis.
