Objective:The purpose of this paper is to report the clinical and radiographic results of THA with an uncemented femoral component after 6-10 years follow-up,and to discuss the medium-term clinical efficacy of these results using an uncemented femoral component and factors effecting their efficacy.Methods:Between 2000-1 and 2003-12,58 cases were operated with the primary total hip arthroplasty in our institution.These consecutive patients followed for an average of 8.2 years (range 6-10) were clinically and radiographically followed up at one and a half month,three months,six months.one year,and yearly thereafter. The purpose is to evaluate medium-term clinical results using an uncemented femoral component. The results of data were analyzed by SPSS 16.0 software,P<0.05 was as statistically significant difference.Results:we followed 58 patients who had had THA with an uncemented femoral component,one femoral component required revision for aseptic loosening of the acetabulum and one for sepsis.After 5 years,one case suffered moderate pain after surgery,three suffered slight pain who were observed different degrees osteolysis around prostheses by X-ray.Complete clinical and radiological follow-up was collected in the 58 patients.Clinically,the Harris hip score improved from a mean of 54 before to 88.3 at the latest follow-up,32(55%) were excellent,22(38%) were good,3(5%) were fair and one(2%)was poor. Radiological analysis showed that 48 hips(83%) had fixation by bone ingrowth,10(17%) were stable fibrous ingrowth and nobody showed unstable fixation with aseptic progressive development.All patients with low Harris score had received prostheses which were undersized in the intramedullary canal,these differences were statistically significant(P<0.05).Osteolysis of the femoral cortex was seen in 4 patients without progressive development. The medium-term clinical efficacy with an uncemented femoral component is similar with those achicved with modern cementing techniques.Conclusion(1). The THA with an uncemented femoral component has better medium-term clinical follow-up results.(2). It is the key to good clinical results that accurate prediction of preoperative femeral prosthesis type and size,the standard surgical procedure and indication of the correct choice.
