The Clinical Observation Of One-visit Root Canal Therapy In The Treatment Of Chronic Apical Periodontitis With Sinus

Objective:To evaluate the clinical effect of one-visit Root Canal Therapy(the combined application of K3 Ni-Ti Rotary files for root canal preparation and Metapex paste for root canal obturation) in the Treatment of Chronic apical Periodontitis with sinus.Methods:10 teeth with Chronic apical Periodontitis with sinus were collected. With careful analysis of symptoms,diagnostic tests,clinical exams, preoperative radiograph,an accurate diagnosis was made. Every tooth of 10 cases were performed by one-visit Root Canal Therapy.Remove all the caries and leaky restoration,clear the pulp chamber,locate the root canal orifice,gain streight line access,use a stainless steel file no.10 with an exploratory curve to creat a patent pathway.Then the crow-down technique could be employed using K3 Ni-Ti Rotary instruments in conjunction with the antimicrobial irrigation solution. Interappointment medication was neglected,at the same time,root canal filling was complished with Metapex paste which was a commercial paste containing calcium hydroxide and iodoform that is available in premixed form in syringes with disposables tips and had a consistent disinfection of the canal.Coronal restoration was placed during that visit as well.The short-term clinical effects were evaluated by recording occurrence of postoperation pain 3 days after treatment and whether the sinus healed 2 weeks after treatment.The long-term prognosis was evaluated according to symptoms,clinical exams and radiographic follow-up findings 6 months after treatment.ResuIts:Result 1:one of 10 cases showed mild postoperation pain 3 days after treatment,others had no pain.Result 2:All the sinus of 10 cases healed 2 weeks after treatment.Result 3:The long-term clinical effects were evaluated 6 months after treatment,(1)All of 10 cases have no symptoms and have a good function;(2)All of 10 cases have no percussion pain;All the sinus healed.(3)Radiographic follow-up findings:periapical radiolucency of 6 cases disappeared; periapical radiolucency of 4 cases became smaller.Conclusion:The chronic apical periodontitis with sinus is treated with one-visit root canal therapy by using K3 Ni-Ti Rotary instruments together with Metapex root canal filling paste,which can both control the canal infection and prevent the microbes from repopulating the root canal space effeciently.That are good for reducing the occurrence of EIAE(endodontic interappointment emergency) and having a good long-term prognosis.
