Objective:To compare laparoscopic cholecystectomy (LC) combined with laparoscopic gallbladder preserving treatment of biliary gallbladder polypoid lesion effect analysis, evaluation of endoscopic polypectomy of gallbladder bile ensure the feasibility and clinical value.Methods:Retrospective analysis of Xinhua Hospital, Dalian University, since October 2007~October 2009 during the 80 cases of gallbladder polypoid lesions. By surgical procedure were divided into 2 groups:①laparoscopic cholecystectomy group of 52 cases;②Paul laparoscopic gallbladder surgery combined with choledochoscopy 28 cases were compared operative time, blood loss, the treatment success rate, gastrointestinal function recovery time, postoperative complication rate, postoperative hospital stay, hospital costs, and gastrointestinal symptoms were followed up after discharge.Results:Both operations were successful.80 cases of resected CPC diameter> 0.5cm polyps 122 and pathology reports:56 cases of cholesterol polyp,70%; adenomatous polyp in 16 cases,20%(of which three cases of proliferative activity); inflammatory polyps in 8 cases 10%. LC,52 cases: The average operation time of 37±18.1min, mean blood volume of 32±16.2ml, postoperative recovery of gastrointestinal function backward flow of fresh low-fat half days of 1.0±0.55d, the number of days after defecation 1.44±1.16d, surgery of no bleeding, bile leakage and infection, wound healed well, postoperative hospital stay 2.5±1.0d, the total cost of 9126±136 Paul gallbladder group:to take in both the one-time net polyps, no lesions left, mean operative time 45±12.6min, mean blood loss 20± 7.5ml, postoperative recovery of gastrointestinal function backward flow of fresh low-fat half the number of days 1.0±0.48d, days after defecation 1.45±0.85d, postoperative hospital stay 2.4±1.3d, the total cost of 9053±173. Of which 1 patient had minor bile fistula cured by conservative treatment. Two groups of patients in the non-research factors such as age, sex ratio, preoperative chronic diseases combined medical conditions, size and number of polyps, the average total operation time, average length of stay, total cost of surgery was no significant difference (P> 0.05), two hand blood loss, postoperative recovery time of diet P<0.05 was statistically significant. The incidence of postoperative complications was no significant difference (P> 0.05), group 2 patients after discharge 74 patients were followed up for an average of 16.4 months, the incidence of gastrointestinal symptoms after long-term statistically significant, were followed up There were no cases of biliary fistula after discharge or polyps back to life, with no deaths.Conclusion:The laparoscopic treatment of biliary polypoid lesions of gallbladder gallbladder and gallbladder function in the protection of the premise of trauma, rapid recovery, low complication rate, treatment certainly, without increasing the cost of the benefits of hospitalization, is feasible in some cases is a new security gallbladder procedure, and worthy of promotion.
