Objective To investigate the prognostic factors of hepatocellular carcinoma.Methods This study retrospectively analyzed the surgical outcomes of HCC in 174 patients who underwent hepatic resection between 2002 and 2005 in Cancer Hospital of Chinese Academy of Medical Sciences. Post-resection prognostic factors were assessed using a univariate log-rank test and a multivariate Cox proportional hazards model.Results The 1 year,3 years and 5 years total survival rate of 174 patients were 93.6%, 85.4%,55.9% respectively. Sex, age, tumor size, resection margin, HBsAg, anti-HCV, cirrhosis degree, et al. is analyzed respectively. Univariate analysis indicates that there is a negative relationship with non-hepatitis infection, severe cirrhosis, vascular invasion, tradition surgical method, recurrence, transfusion, poor histologic differentiation for overall prognosis. Multivariate analysis showed that cirrhosis degree, anatomical surgical method, recurrence occurrence was the most important prognostic factors for long term outcome. No significiant differences in age, lymphocytes infiltration, TNM Staging were existed in the study group.Conclusion Hepatitis virus infection is found to be an additional good prognostic factor. Cirrhosis degree and anatomical surgical method are the most important prognostic factors for long term outcome. Liver background is so important for long term outcome that antiviral treatment is the best choice to alter the natural history of hepatocarcinogesis by modulating fibrosis and cirrhosis.
