Research On The Situation Of Long-term Home Oxygen Therapy And Its Influencial Factors Of Chronic Obstructive Pulmonary Disease Patients

Background:Long-term home oxygen therapy (LTHOT) is a main treatment to relieve the progress of Chronic Obstructive Pulmonary Disease (COPD) and to improve the patients'prognosis. More and more patients have received LTHOT. In the process of oxygen therapy, the patient's correct use of LTHOT is important to ensure the effect of LTHOT. Some studies showed that the situation of LTHOT in COPD patients was poor as a whole. Only knowing its influencial factors can we help the patients to receive effective LTHOT. Therefore, it is necessary to explore the situation of LTHOT and its influencial factors.Objecctive:To describle the situaiton of LTHOT in COPD patients and to explore the influencial factors that affect their performance of LTHOT.Method:A descriptive design was used. By convenient sampling, inpatients from four general hospitals in Beijing were recruited in this study. Self-designed Questionnaire was used to collect information about COPD patients'situation of LTHOT performance, the impact from medical staff, comfort level of LTHOT, the influence of LTHOT to patients' life and the accessibility of oxygen resources. Self-designed LTHOT knowledge questionnaire was used to evaluate the level of COPD patients'knowledge. Self-efficacy scale, Family support scale, Hospital anxiety and depression scales were used to collect information about other factors that could affect the performance of LTHOT in COPD patients.Results:After doctors'advise,97.4% patients performed LTHOT in 116 participants, only 15.5% of whom reported that they were fully performed LTHOT. The main problem in LTHOT was the duration of LTHOT, only 19.8% patients insisting on inhaling oxygen for 15h and more per day, there were also some problems in the process of performing LTHOT, such as the duration and distribution of time to perform LTHOT, the selection of flow oxygen and effect monitoring. The influencial factors to the performance of LTHOT for COPD patients included LTHOT knowledge, definite oxygen therapy project, the influence of LTHOT on patients'images, disease severity, acquire oxygen resources conveniently and the comfort level of inhale oxygen, these factors explained 96.2% of the variance of performance to LTHOT.Conclusions:The situation of LTHOT in COPD patients was not good. There were some problems existing in the process of LTHOT, healthcare providers needed to take effective measures to improve the situation of LTHOT. Healthcare providers should pay attention to severity patients, provide LTHOT knowledge, definite oxygen project, select suitable oxygen supply equipment for them, improve patients'comfort level, and provide oxygen resources to improve situation of LTHOT effectively.
