Objective:The study mainly through the investigation of the number and structure of health manpower in Gansu province's medical institutions, and through the investigation of the health services utilization and medical education and training in Gansu Province to understand the distribution of health human resources in Gansu Province's medical institutions, to identify the main problems in the health human resources, to put forward corresponding suggestions and countermeasures. Analyse the balance of supply and demand in health manpower to provide scientific basis for the functional departments to establish a reasonable health manpower development plan, to strengthen the building of health personnel, adjust to the direction of medical education, and construction of health policy. Established the health system which adapt to economic and social development in Gansu, in order to meet the growing demand for medical and health services, to promote the healthy development of medical and health undertakings in Gansu.Methods:Taking multi-stage stratified cluster sampling principle for the investigation unit, according to the type of economic and social development is divided into three areas (city, state); then extracted a county and a district from every kind region in the middle of social and economic development; and extracted corresponding three township and three streets from the county and district. The provincial and the extraction of city (state) and county (district) belongs to medical institutions all were included in the survey, and the street community health service center (station) and rural township hospitals and village clinic were included in the survey. Investigation mainly include medical institutions'health services utilization, and health manpower's education degree, professional title, occupational category, post, composition of all kinds of health technicians, and the enrollment scale and employment at various levels of medical colleges in Gansu province. On-site investigate the survey units and individuals through the corresponding questionnaire. According to the survey data, statistical describe the basic situation of the health manpower to understand the distribution of the health manpower; taking the health services demand approach measure health manpower demands in different types of area and various medical institutions in Gansu province; and combined enrollment scale and employment in medical colleges in recent years to analysis the balance of the health manpower's supply and demand from their own of medical education and training.Results:1. In the quantity:at present, the medical institution's hygienic and technical personnel ownership is 2.45-3.47/thousand, the demand is 3.19-4.09/thousand, the amount of hygienic and technical personnel is insufficient.2. In the structure:in proportion of male and female health workers is 1:2 in Gansu province's medical institutions. Health workers age composition in young and middle-aged, spindle distribution, age structure is reasonable.The health personnel of provincial medical institutions is with bachelor degree of 33.5%, medical institutions of the class one and two city and county are mainly with college degree, and medical institutions of the class three city and county(district) and all kinds of township (street) and village in secondary education. Overall, the health personnel's education level is low in Gansu province.Overall, hiring hygienic and technical personnel in medical institution is a large proportion, and hiring hygienic and technical personnel in medical institutions of the class one and two township (street) is relatively large, and the proportion of the organizational and hiring hygienic and technical personnel is respectively 1.2/1 and 1.4/1. The proportion of provincial medical institution's professional title is 1:2:3, primary professional title is short. The medical institutions'Senior professional title is less in medical institutions of class one and three city (1:4:4,1:6:6). The medical institutions'primary and middle-rank professional title is high grade in the various areas county (district) (1:4:7,1:6:9,1:6:11). The title ratio is imbalance in class one and three township (street), primary title personnel is high proportion (1:17:57, 1:15:7).The proportion of doctors and nurses is 1:3 in provincial medical institutions, nurse is insufficient. The proportion of doctors and nurses is reasonable in municipal medical institutions, medical institutions'nurse is insufficient in class three county (district) (1:9), medical institutions'nurse is obviously insufficient in class one and three township (street) (1:0.6,1:4).The proportion of general practitioners physicians is respectively 0.1%,0.2%, 0.1% and 0.4% in provincial and municipal medical institutions. Class one county (district) is 2.1%, and class two and three county (district) is no general medical practitioners. Class one and two and three township (street) is respectively 4.0%, 5.3% and 5.6%.In rural doctors hold proportion, in medical institutions of the class one and two and three village, certificate of rural doctors in proportion is respectively 78.6%, 92.6%and 68.7%.3. In the current medical education and training of medical colleges in Gansu province:the enrollment scale of undergraduate medical student declines year by year, the enrollment scale of secondary hygiene school increased year by year, but the increase rate decreases. The college and secondary school enrollment in Gansu province are from the province, but employment proportion outside the province increases year by year. The percentage of admission in medical colleges outside the province increases every year, the proportion of graduates outside the provinces along the flow also increased, lead to train health professionals in Gansu province loss. If the scale of development under the current enrollment, the medical students in Gansu province is gradually coming years can not meet the future demand of health manpower in Gansu province.Conclusion:The number of the health workforce of Medical institutions in Gansu cannot satisfy the current number of demands. The education degree and professional title of health personnel is low in the overall level, the post and professional structure is prepare adjustment, the medical students training and health personnel demand does not adapt. Suggestions for moderate to increase human resources in Gansu province's medical institutions, especially the health human resources in rural areas and the areas which economic level are relatively backward. Suggested by regional health manpower planning, rational configuration of human resource, taking a various ways to introduce, encourage and train medical personnel. The formation of high-quality, stable echelon of health personnel to meet the increasing development of medical and health service demands in Gansu province.
