Objective:To investigate the expression of osteopontin(OPN) and matrixmetalloprotei-nase-2(MMP-2)and vascular endothelial growth factor(VEGF)in the ectopic and eutopic endometrium from woman with endometriosis, thereby to explore the role of them and their relationship in the pathogenesis and the signs of Traditional Chinese medicine of endometriosis. To study the relations between the serum levels of OPN and Ca125 in patients of endometriotic and to discuss its value of diagnosis and clinical applicationMethods:The expression of OPN,MMP-2 and VEGF are detected by immunohistochemistry method in 32 case of ectopic endometrium and 26 case of ectopic endometrium from women with endometriosis,and 23 case of specimens of endometrium from women without endometriosis; the levels of osteopontin in serum of two groups were detected by enzyme-linked immunosorbent assay (ELISA), and Ca125 in serum of patients with endometriosis were detected by radio immunoassay(RIA). We also detected the OPN expression of 26 case of ectopic endometrium from women with endometriosis in the past in order to analysis their correlation and recurrence.Results:1. The expression of OPN in the patients with endometriosis's ectopic and eutopic endometrium were higher than that in control group; OPN in the eutopic endometrium was lower than that in ectopic endometriosis too. There were no difference of OPN expression in study group and control group between the two phase of menstrual cycle. There was asignificant difference in levels of OPN in the serum of study group between proliferative stage and secretory stage; however, the levels of OPN had no relationship with menstrual cycle in the serum of control group. The OPN expression in ectopic endometrium and in the serum of study group had statistic correlation with the division of AFS-r, but it had no relationship in eutopic endometrium. There was no change of preoperative and postoperative in the serum OPN levels of two groups.2. The expression of MMP-2 in the patients with endometriosis's ectopic and eutopic endometrium were higher than that in control group; MMP-2 in the eutopic endometrium was lower than that in ectopic endometriosis too. The expressions of MMP-2 in the study group and control group had no relationship with menstrual cycle. The MMP-2 expression in ectopic endometrium had statistic correlation with r-AFS staging, but it had no relationship in eutopic endometrium.3. The expression of VEGF in the patients with endometriosis's ectopic and eutopic endometrium were higher than that in control group; VEGF in the eutopic endometrium was lower than that in ectopic endometriosis too. The expressions of VEGF in the study group and control group had no relationship with menstrual cycle,and no assoeiated with r-AFS staging.4. There was a significantly positive correlation between expression of OPN and MMP-2 or VEGF in endometriosis.5. The Ca125 lever in the serum of study group had no relationship with the OPN lever.6. The expression of OPN in the recurrent group was significantly higher than that in the unrecurrent group.7. The OPN expression in ectopic endometrium and in the serum of study group had no statistic correlation with the three types of Chinese medieal (TCM) syndromes in endometriosis,but both of them are higer than those in control group.Conclusion Overexpression of OPN and MMP-2 or VEGF may play an important role in the transformation,development and prognosis of endometriosis. They may make synergistic effect in angiogenesis,local invasion and distant metastasis. The expression of OPN had statistic correlation with the division of AFS-r and prognosis of endometriosis. It has a potential clinical significance as a prognostic marker for patients. The relations between OPN and TCM syndromes were expected to provide a new diagnosis and treatment of direction.
