The Evaluation On Pelvis Free Liquid Indicated By Ultrasound Which Diagnosis The Quantity Of Blood In Abdominal Cavity Of Ectopic Pregnancy Patient

IntroductionEctopic pregnancy is one of the common acute abdomen.In recent years, accordsing to report of literature:The rate of ectopic pregnancy was obviously increasing.The American Disease Control and Disease Prevention Center reported: during 1970-1992,the rate of ectopic pregnancy increased 4 times.About 11000 women suffered from ectopic pregnancy(1-2% of total pregnancy people).The ectopic pregnancy is the top of emergency.It is also the cause of pregnant women death.So early diagnosis and treatment are important for ectopic pregnancy patient.At resent the diagnosis of ectopic pregnancy depends on Ultrasound and Human Chorionic Gonadotropin.The pelvis free liquid is important to evaluating the patient state.We try to estimate the actual quantity of blood in abdominal cavity through the pelvis free liquid indicated by ultrasound,and provide more accurate treatment evidence for ectopic pregnancy patient.We also could provide the reference for emergency operation. The pelvis free liquid indicated by ultrasound indicate the quantity of blood in abdominal cavity of ectopic pregnancy patient.It will provide an important evidence for the treatment of ectopic pregnancy patients.Materials and Methods1. The selection of case:The patients were in hospital in ShengJing Hospital and operated in time.Age between 22-45,herght between 155cm-170cm,weight between 45kg-55kg.2. The patients examined by ultrasound before operation.The ultrasound machine is GE730 made in American. 3. Examine methods:The patients take prostration,and examined by abdominal probe which is 5-9MHz4. Divide into two groups according to the the pelvis free liquid indicated by ultrasound(X):less than 3cm group,3-5cm group.We found the corresponding the actual quantity of blood in abdominal cavity(Y) through the record of operation.5. Statistical analysis the date by SPSS 16.0 software.ResultThe 243 cases in the less than 3cm group fit the linear regression equation: Y=189.155X-194.099.According to the hypothesis test of linear regression, the quanaty of free liquid in pelvis prosess linear relation with the actual quanaty of blood in pelvis (t> t0.05/2 (241) p<0.05.ata=0.05 level,refusing null hypothesis).So the regression coefficient has statistical significance. So doctors could calculate the actual quanaty of blood in pelvis by this equation,when the quantity of free liquid in pelvis is less than 3cm. The 257 cases in 3-5cm group fit the linear regression equation: Y=501.013X-1206.215.According to the hypothesis test of linear regression, the quanaty of free liquid in pelvis prosess linear relation with the actual quanaty of blood in pelvis (t> t0.05/2 (255) p<0.05.ata=0.05 level,refusing null hypothesis). So the regression coefficient has statistical significance. So doctors could calculate the actual quanaty of blood in pelvis by this equation,when the quantity of free liquid in pelvis is between 3cm and 5cm.Culcalating the actual quanaty of blood in pelvis differently in 95% confidence interval are (315.04,394.04) ml, (504.889,589.775)ml, (754.137, 841.54) ml, and (976.903,1119.788) ml,when we selecte randomly the quantity of free liquid in pelvis 2.9cm,3.5cm,4.0cm,and 4.5cm.ConclusionWhen ultrasound indicates the quantity of liquid in pelvis less than 3cm,the actual quantity of blood in pelvis is less than 400ml. At this time we consider that there is less bolld in pelvis. When ultrasound indicates the quantity of liquid in pelvis more than 4.0cm, the actual quantity of blood in pelvis is between 750-850ml.At this time doctors should vigilance the patient who maybe lost too much blood. When ultrasound indicates the quantity of liquid in pelvis 4cm, the actual quantity of blood in pelvis is among 1000ml.We shoud pay much attention to hemorrhagic shock.
