ObjectiveThe number of T2DM quickly increases currently, study in recent years found that the RYGB has therapeutical effect on obesity T2DM. Early studies found that the adiponectin plays a very important part in the T2DM. In order to study whether the RYGB has therapeutical effect on none obesity T2DM or not, we research the variety of adiponectin level of GK rats go after the RYGB to investigate the RYGB's therapeutical effect and mechanism for T2DM,providing theoretical basis for the surgical operation treatment of T2DM.MethodsThirty eight-week male GK rats were randomized into the gastric bypass group(RYGB group),false operation group (SO group),food reduce group(FR group), age and sex matched Wistar rats were used as control group(C group). At the age of 10 weeks, the rats of RYGB group accept Roux-en-Y gastric bypass, the rats of SO group accept corresponding a digestive tract cut off and fit together, as the same time as RYGB group, the rats of FR group give 1/3 normal food measure(9 g/d) after operation, the rats of C group do not give any interference. Weight and food intake were measured two times one week before surgery,one time per day in the first and second week after surgery, two times per week in the third and fourth week after surgery, weekly average recorded for the week of weight and food intake. Determine fasting blood sugar through vena caudalis with blood sugar instrument one time per week. Test OGTT at the first week before surgery and the third week after surgery, the rats blood sampling time points were 0,30,60,90,120 minute after infuse the stomach. Measure the level of insulin with ELISA at the first week before surgery and the third week after surgery, and count HOMA-IR. Measure the level of blood plasma adiponectin with ELISA when rats sentenced to death at the fourth week after surgery. Results1.The change of weight after surgery.The weight of RYGB and SO declines to the lowest at the first and second week and then rises gradually, but the weight of FR continuously descends. There is no significant differences between RYGB and SO(P>0.05), the weight of RYGB is more than the FR.(P<0.05).2. The change of food intake after surgery.There is no significant difference among each group before surgery(P>0.05), the food intake of RYGB declines to the lowest at the first week after surgery and then rises gradually, start with the second week after the surgery, the food intake of RYGB is more than the FR (P<0.05).3. The change of FBS after surgery.There is no significant difference among each experimental groups before surgery(P>0.05) and experiment group is higher than the C group(P<0.05). At the second to the fourth weeks after surgery, there is no significant differences between RYGB and C (P>0.05), but the FBS of SO group and FR group showed higher than C group.(P<0.05).4. The change of OGTT after surgery.There is no significant difference among each experimental groups before surgery(P>0.05). OGTT curves show significant difference between experiment groups and C group(P<0.05). At the last day of the third week after surgery, there is no significant difference of the blood sugar level after glucose infusing stomach thirtieth minute among experimental groups.(P>0.05). There is no significant difference between RYGB and C after sixtieth, ninetieth, one hundred and twentieth minute of infusing stomach(P>0.05),and then after infusing the stomach, the blood sugar of SO and FR group is much higher than RYGB group (P<0.05).5. The change of FPI after surgery.Before surgery, the fasting blood plasma insulin level of experimental groups rats is higher than C group(P<0.05). At the thirtieth minute after infusing stomach, the level of blood plasma insulin in C group obviously grows up and then the experimental groups sat undulate at the foundation level. After surgery, there is no significant difference between the RYGB and C group in basic plasma insulin (P>0.05). The level of fasting plasma insulin of SO and FR group is obviously higher than C group(P<0.05). there is no significant difference between the RYGB and C group in plasma insulin at the thirties minute after infusing stomach(P>0.05). At the thirties minute after infusing stomach, the level of insulin of SO group is lower than which before surgery(P<0.05) and in the FR group it is higher than which before surgery(P<0.05).6. The effect of RYGB on the HOMA-IRBefore surgery the HOMA-IR of RYGB, SO, FR is obviously higher than the one of C (P<0.05). The HOMA-IR of SO and FR group has no significant difference before and after surgery(P>0.05).The HOMA-IR of RYGB group obviously descends compared with before surgery(P<0.05), there is no significant difference compared with C group(P>0.05).7.The Adiponectin changes after surgery.The level of adiponectin of RYGB group rats is obviously higher than SO and FR group at the fourth week after surgery(P<0.05),and higher than C group(P<0.05).There is no significant difference between SO and FR after surgery,and then it is lower than C group(P<0.05).ConclusionsThe GK rats'blood sugar become lower, the insulin resistance alleviate after RYGB.RYGB have therapetical effect on T2DM of GK rats. The level of adiponectin increment of the GK rats after RYGB may be one of the mechanisms of treatment for T2DM.
