Investigate the value of fluid/serum in the differential diagnosis of malignancy- related and non- malignant ascites.METHODS The level of CA242,CA72-4,CA15-3 in ascites and blood were checked in the same time in 44 patients with malignant ascites and 37 cases with benign fluid,using IMRA.RESULTS When the specificity is 100%,the sensitivity of the three tumor markers in serum,fluid,and F/S respectively is that CA242 :31.82 %,43.19 %,63.63%;CA72-4:47.73%,52.27%,75.0%;CA15-3:36.36%,34.09%,65.91%.Compar to serum or fluid,the sensitivity of every tumor marker in F/S ratio is higher(P<0.05).When two tumor markers were combinated, the combination of CA242 and CA72-4(each F/S ratio >1.2) showed the lowest sensitivity. There were no distinguished difference between CA242,CA15-3(each F/S ratio >1.2) and CA72-4,CA15-3(each F/S ratio >1.2).When the three tumor markers were combinated, the sensitivity did not become higher.Conclusions1 Detect the level of CA242,CA72-4,CA15-3 in serum and ascites limited in the diagnosis of malignancy- related because of the very low sensitivity.2 Using the F/S ratio of the three tumor markers could improve the sensitivity. When CA72-4 and CA15-3,or CA242 and CA15-3 were combinated, the sensitivity was significanthly improved.3 When the three tumor markers were combined totally,the sensiiticity was not better than the combination of CA72-4 and CA15-3,or CA242 and CA15-3.
