| Objective:Explore intervention efficacy and the main complications by reviewed and analysis of 1025 congenital heart disease patients before July in 2009, to improve the level of congenital heart disease intervention.Methods:Confirmed by X-ray, angio cardiography and transthoracic echocardiography(TTE),transcatheter closure of arterial septal defect (ASD), patent ductus arteriosus(PDA), ventricular septal defect(VSD),and percutaneous balloon pulmonary valvuloplasty(PBPV) treatment of congenital pulmonary stenosis (PS). TTE and ECG examination were applied to evaluate efficacy and complications after the procedure.Results:(1)Interventional therapy of 1025 congenital heart disease patients, the total success rate of the procedure was 98.8%(1013/1025). The main complications developed in 12 cases (1.17%) and the mortality was 0.1% (1/1025). (2) In 369 ASD group, the success rate of the procedure was 97.6% (360/369). Compared with preoperative LVEDD increased significantly (P< 0.01), LVEF increased (P< 0.05), RVD decrease (P< 0.01) postoperative 24-72h and postoperative six months. Compared with postoperative 24-72h LVEDD increased significantly (P< 0.01), RVD decrease (P< 0.01) postoperative six months and LVEF not statistically significant differences(P> 0.05). Postoperative six months, one year, two years, three years compared with each other, LVEDD, RVD and LVEF not statistically significant differences(P> 0.05). main complications occurred in 5 cases (1.63%), included 3 cases of device embolization,1 case of cerebral thrombus embolization,1 cases of cardiac tamponade. (3)In 261 PDA group, all patients were implanted occluders successfully. Compared with preoperative LVEDD and LVESD increased (P< 0.01) postoperative 24-72h and postoperative six months. Compared with postoperative 24-72h LVEDD and LVESD increased (P< 0.01) postoperative six months. Postoperative six months, one year, two years compared each other, LVEDD and LVESD not statistically significant differences(P> 0.05). The main complication occurred in 1 cases (0.38%) which was iliac vein injury.(4) In 289 VSD group, occluders were implanted in 286 patients successfully. The success rate of the procedure was 99.3%. Compared with preoperative LVEDD increased(P< 0.01), postoperative 24-72h and postoperative six months. Compared with postoperative 24-72h LVEDD increased (P< 0.01) postoperative six months. Postoperative six months, one year, two years, three years compared with each other LVEDD not statistically significant differences (P> 0.05). The main complications occurred in 5 cases(1.73%), included 3 cases of hemolysis and 2 cases of III degree atrioventricular block (AVB).(5)In 106 PS group, the success rate of the procedure was 99.1%.50 cases were followed up,△P and RVD decreased significantly,compared with preprocedure (P<0.01). The main complication occurred in 1 case(0.94%) which was acute tamponade. The mortality was 0.94%(1/106).Conclusions:(1) Interventional congenital heart disease is a safe and effective therapy. (2) The complications rate was slightly higher in the interventional VSD and ASD.We should pay attention to the postoperative care and follow-up to management complications in time. |
