Backgroud:Low back pain is a common clinical symptom of troubled people in modern society degenerative disc disease (DDD) is the most common cause of low back pain caused. With the aging of the human intervertebral disc degenerative changes accordingly, it was considered inflammatory mediators have also participated in lumbar disc degeneration factors, thus inhibiting the participation of inflammatory mediators, inflammatory mediators due to lumbar disc degeneration may be caused by a certain delay effect on the future treatment or prevention of degenerative lumbar disc and possible to provide a direction. Inflammatory mediators in the upper regulatory protein, there are many, including a p38MAPK is an important mitogen-activated protein kinase (mitogen activated protein kinase, MAPK) signaling system is the major intracellular signal transduction system, p38 pathway involved in cell growth, developmental and cell synchronization between the function of a variety of physiological processes, and with inflammation, stress response is closely related to their involvement in the regulation of TNF-α, IL-1, COX-2, IL-6, IL-8, HMGB1 and other inflammatory stress factor. These inflammatory factors are considered among disc degeneration of important reasons. Cells, transmission of information is considered a meeting point and common pathway. Therefore suggested that by inhibiting the p38 pathway to delay the degeneration of the discs is a future direction.Objectives:1. According to P38MAPK expression of content and varying degrees of lumbar degenerative disc between the MRI imaging of the statistical comparison, guess P38MAPK expression of content and varying degrees of degenerative diseases, the relevance of MRI manifestations. Indirectly probe the involvement by the P38MAPK mediated inflammatory process caused by the relationship between lumbar disc degeneration.2. P38 inhibitor based treatment or delay the image changes according to the early to judge mild disc degeneration between the effect of this treatment approach to provide the feasibility of support.3. Based on imaging findings to develop conservative treatment of degenerative lumbar disc herniation and other spinal diseases, the use of a variety of treatment programs to provide a p38 inhibitor treatment theory.Method:Collection of Beijing Military Region General Hospital Spinal Unit November 12,2009 to February 2,2010 lumbar disc herniation and lumbar spinal canal stenosis and lumbar spondylolisthesis 37 cases of lumbar disc surgery, as well as the control group 9 cases, the use of tissue protein measurement commonly used in Western blotting measurement of fibrous ring of P38 MAPK content. Preoperative imaging MRI according to different levels of disc degeneration between the grading and grouping, MRI images according to (Pearce grade) grouping. According to MRI were divided into three groups for their different levels in order to involve inter-plate of fresh fractures of the lumbar spine in patients with the control group. Tests compare the difference between the two groups, respectively, t-test. Speculated that MRI in grading and intradiscal levels the relationship between expression of P38 MAPK.Results:Since November 12,2009 2010 February 2 JCP collected 37 cases of degeneration of the discs, which Pearce gradeⅢ-class group of five cases of degeneration of the discs, Pearce gradeⅣ-level tray 8 cases between the two groups, in addition to the blank control group and Pearce gradeⅢlevel, P> 0.05 not statistically significant, the blank control group and Pearce gradeⅣ-level group and gradeⅤlevel compared Pearce, all P<0.05, significant statistical difference. Pearce gradeⅢgradeⅣgrade-level group and the Pearce group P<0.05 was statistically significant statistical difference between the two groups., Pearce gradeⅣ-level group and compared Pearce gradeⅤlevel, P<0.05 was statistically significant statistical difference between the two groups.Conclusion:P38 MAPK in intervertebral disc degeneration in the MRI classification Pearce different manifestations. Degenerative disc grading between heavier light-level expression of the degeneration of high content of the discs, suggesting that disc degeneration according to MRI Pearce may be the initial classification to determine the expression level of P38 MAPK. And mild disc degeneration among the use of P38MAPK inhibitors delay the degeneration of the discs that provide a possible future treatment options.
