Objective:At present,there have been studies devoted to revealing the scientific connotation of the treatment of intervertebral disc degeneration from the aspects of biomechanics and biochemistry.Therefore,the mechanism of action of Tuina therapy remains to be elucidated.In this experiment,a model of disc degeneration in white rabbits was established to explore the mechanism of Tuina on disc degeneration.By detecting the expression levels of TGF-?1,inflammatory factors and autophagy genes in degenerated intervertebral discs,and the changes of TGF-?1,IL-6,TNF-?,Beclin1 and LC3 after Tuina intervention,the aim is to provide scientific basis for the treatment of lumbar disc degenerative diseases by Tuina.Method:This experiment consists of two parts.First,a comparative study of rabbit lumbar disc degeneration models was carried out.A white rabbit was randomly selected to observe the structure of the lumbar intervertebral disc.Then,the experimental white rabbits were divided into a blank group,a puncture group and an upright group.The rabbits of puncture group was modeled with puncture fiber rings.In the upright group,the white rabbit was erected on a round tube for modeling.After 4 weeks,the general situation of the white rabbit,the anatomical structure of the intervertebral disc,and the histological changes of the disc during HE staining were observed.According to the experimental results,the modeling method is confirmed to carry out the following experiments.Second,after the modeling method was determined,30 white rabbits were randomly divided into a blank group,a model group and a Tuina group,with 10 in each group.Then,the rabbits in the model group and the Tuina group were subjected to intervertebral disc degeneration.After modeling,the Tuina group conducted a 6-week Tuina intervention once a day for 10 minutes each time.And no special treatment is performed for the blank group.After 6 weeks,the white rabbits were killed by air embolization,and the discs were taken.Finally,HE staining was used to observe the morphological changes of intervertebral discs.ELISA and PCR were used to detect the expression of TGF-?1 protein and genes in the intervertebral discs,as well as the expression of inflammatory factors IL-6 and TNF-? and autophagy-related genes Beclin1 and LC3 expression.Results:1.The rabbit disc degeneration model experiment showed that the histological score of the disc in the puncture group was obviously higher than that in the blank group,and the difference was statistically significant(P <0.05).2.Compared with the blank group,the protein expression and gene expression of TGF-?1 in the model group were obviously increased,and the difference was statistically significant(P <0.05).3.Compared with the blank group,the expressions of inflammatory factors IL-6 and TNF-? in the intervertebral disc of the model group were significantly increased,and the difference was statistically significant(P <0.05).4.Compared with the blank group,the m RNA content of autophagy genes Beclin1 and LC3 in the intervertebral disc of the model group increased,and the difference was statistically significant(P <0.05).5.Compared with the model group,the expressions of TGF-?1,IL-6,TNF-?,and Beclin1 in the intervertebral discs of the Tuina group showed a downward trend.The expressions of TGF-?1 and TNF-? and Beclin1 were statistically significant(P <0.05).Conclusions:1.Puncture of the intervertebral disc fiber ring promoted the degeneration of rabbit lumbar disc.2.TGF-?1 and inflammatory cytokines IL-6,TNF-?,and autophagy gene Beclin1,LC3 are involved in the intervertebral disc degeneration.3.Tuina may participate in intervertebral disc degeneration by regulating the expression of TGF-?1,inflammatory factors and autophagy genes.
