| Research background:The cancerous ache (i.e. cancer pain) is one of the most common and unbearable symptoms of malignant tumor. It reduces survival quality of patients with cancer from the psychologal, the physiologal, the mental damage and so on. Therefore the treatment of pain is the main cancer treatment. WHO has said that the effective treatment of cancer pain was one of the four unified plans with tumor. Although the three-step analgesic pain has been internationally recognized cancer pain methods of treatment, yet has not been able to control all cancer pain ideally. And The opioid analgesic drugs have side-effects such as drug addiction, the patient tolerance of pain and so on. Function of the Chinese medicine has been approvalled in cancer pain treatment by many year research and practice. External TCM is an important component of Chinese medicine. Comparison of western medicine, Chinese medicine External analgesic edge effect is more reliable, more safety, less toxicity, no addiction and no withdrawal. Therefore it is widely used clinically.Research purpose and meanings:The objective of this study, from clinical Angle, was to evaluate the effect of zi-ni-zhi-tong in the treatment of cancer pain, and provide a clinical practical data for TCM external method with controlling cancer pain. TCM external method may be an effective mean of relieving pain, and one of the development directions in the treatment of TCM therapy for pain. Research methods:Use prospective randomized controlled clinical study, establish diagnostic criteria for sample, inclusion criteria, exclusion criteria and standards for the suspension of clinical trials. Observation of 60 cases of hospitalized patients were randomly assigned to treatment group 30 cases,30 cases of the control group, control group patients to painkillers, moderate pain (4-6 points):sustained-release tablet of tramadol (100mg/ tablet), a 1 film, Q12H, even for 10 days served; severe pain (7-10 points): morphine sulfate controlled-release tablets (30mg/tablet), a 1 film, Q12H, even for 10 days served; treatment group based on the above, Plasting zi-ni-zhi-tong on pain spot, once a day, total 10 days. Records of pain intensity, and times Of breakout pain before and after treatment to observe the daily life of self-care ability and ADL score scale, and statistics. Research results:Compared average total curative effect about pain score of two groups patients. The total effective rate of group with zi-ni-zhi-tong plaster was 96.7%, while control group was 83.3%, result that two groups total efficiency was tested by the X2 was no significant difference (P> 0.05). The efficiency of the treatment group was 70%, but the control group was 33.3%. Compared efficiency of the two groups, the X2 test, P<0.05, was statistically significant difference; that the two groups to improve the efficacy of pain there was no significant difference between the rate, while the rate a significant difference in the effect of statistical significance. The total effectiveness, Patterns of qi-ji-yu-zhi,patterns of xue-yu-du-jie,patterns of two-mixed in.the treatment group, respectively was 26.7%,23.3%,46.7%;while in the control group was26.6%,20%,36.7%. Effectiveness with patterns of two-mixed in the treatment group was higher than control group. That was, zi-ni-zhi-tong plaster may play a certain auxiliary role stopping cancer pain with the pattern Of qi stagnation and blood stasis, even may stronger than control group.Times of eruptive pain comparison:the times of two groups at the beginning of the therapy were tested by t test, there was no significant difference (P>0.05);while at the end of the therapy, there was significant difference between before and after treatment in the zi-ni-zhi-tong plaster group; but no significant difference in the control group. There was the same result when observing the average number of using tramadol at the beginning and end of the therapy. This study showed zi-ni-zhi-tong plaster with medicine could effectively reduce the occurrence of eruptive pain, and improve patient's survival quality.Zi-ni-zhi-tong plaster group, control group comparison of quality of life before and after the therapy by t test, P, respectively,<0.01, were statistically significant differences. This noted that quality of life were significantly improved by the both methods. Before and after the points of difference between the two groups by T test, P<0.05, two treatments that improve the quality of life of patients with a difference. This shows that the two groups to improve the treatment of pain in the difference between the treatment group of patients with improvements in the quality of daily life than the control group still improve.Research conclusion:Zi-ni-zhi-tong plaster could effectively relieve cancer pain, reduce times of breakout pain, improve quality of life with cancer patients, so it could be used as one of the means to control cancer pain on Clinic.
