| ObjectiveBased on the dialectical thinking of clinical in existing,the differential measurement mathematical model as well as the Clinical Diagnosis and Treatment System of TCM, this paper propose a new kind of "Syndrome Differentiation Unit" model of thought, and establish the corresponding mathematical model and algorithm, and develop TCM clinical syndrome differentiation system. By extraction samples of clinical cases to test the system, to verify the accuracy of "Syndrome Differentiation Unit" model, and explore the factors affecting diagnostic accuracy of it.MethodsAs the basic characteristics of TCM dialectical thinking as the base, as the syndrome elements differentiation system as the theoretical, establish the basic framework for "Syndrome Differentiation Unit" Measuring diagnostic model, and describe the concept,basic properties,syndrome elements attribute,operation rules,syndrome type determining method of "Syndrome Differentiation Unit" model; On this basis, to build mathematical model assumes, to establish the weight quantitative method of Syndrome Differentiation Factor,"Syndrome Differentiation Unit",syndrome type, and to establish the basic computing model of "Syndrome Differentiation Unit", "Syndrome Differentiation Tree" algorithm; As the diagnostic knowledge of《Diagnostics of TCM》as reasoning based, to construct dialectical knowledge database, and develop TCM Clinical Diagnosis System "Hong Mian I". By random sampling from the Guangzhou University of TCM First Affiliated Hospital, four hospitals in a total sample of 850 cases of clinical cases, by collectting patients in general (name,gender,age,treatment time), as well as the main symptoms,signs,syndrome type, to establish the Clinical Medical Records Sample Database,and test the Clinical Syndrome System "Hong Mian I". The use of statistical methods analyzing and comparing Syndrome Distribution,Confidence Coefficient,Distinguishing Coefficient,Solutions Coefficient,The Rate Of Matching,Accuracy and other test indicators of the four groups of samples.ResultsThe core idea of the "Syndrome Differentiation Unit" model will be organic combined to syndrome differentiation unit which can be able to represent the character of Syndrome Diagnosis from the syndrome. It also reflects the Disease-Position and the Disease-Properties which are two basic elements of dialectical, with the basic nature of Complete,Uniqueness,Holistic,Equivalence. It permit a change in type of weight through "overlay" and "inspire" unique operation rules, and determine the diagnosis of Syndrome by Comparing the total weight of the Syndromes. " Syndrome Differentiation Unit" mathematical model justify the syndrome elements attribute as coordinate system, and transform syndrome elements's nature of syndrome to Multi-Variable Data Structure of Multi-Dimensional And Advanced, and establish the " Syndromes-Syndrome Differentiation Unit-Syndrome Type" semi-quantitative model of dialectical reasoning and "Reverse N-Ary Tree," differentiation algorithm. Clinical cases testing indicate, The overall accuracy rate of the model reaches up to 95.1%, consistent rate is 93.6%, the reasons for not match the original medical record in sample miscarriage of justice:system dialectical error accounted for 4.9%, the original medical record dialectical error (misdiagnosis) accounted for the proportion of 0.7%; other reasons accounted for 0.9%of the overall sample; the reasons for determine wrongly in sample miscarriage of justice:the credibility factor which is lower than the threshold, accounting for 2.2%, distinguish coefficient which is lower than the threshold, accounting for 2.3%, other reasons accounted for 0.3%of the overall sample.Conclusion"Syndrome Differentiation Unit" model proposes a new diagnostic method of measuring the dialectic, and constructes the mathematical model and reasoning algorithms that reflect the mapping relations between Syndrome Differentiation Unit and Syndrome Type, and simulates TCM clinical syndrome differentiation thinking intuitively to a certain extent; Clinical cases testing indicate, "Syndrome Differentiation Unit" model of clinical syndromes with a high accuracy, the results also show that the main reasons for leding to syndromes do not match with the original medical record diagnosis is the model's own dialectic errors, and the major factor for affecting the model itself is syndrome type ambiguity as well as the dialectic insufficient information. |
