| Object ive1. To evaluate the treatment effect of tong guan capsule joint Bone marrow mesenchymal stem cells on Cardiac function after acute myocardial infarction.2. To evaluate the clinical treatment effect of tong guan capsule on cardiac function and quality of life after acute myocardial infarction.Methods1. The animal researchAn AMI model is built by the left coronary artery ligations on 60 healthy prague dawley rats (avoirdupois:200±20g).40 surviving AMI of them were divided randomly into four parts:①stem cells and Tong Guan capsule treatment group(n=10):After 24 hours of AMI preparation,0.5 ml (2×107) stem cells was mainlined,2 Tong Guan capsules were given per day;②stem cells group(n=10): After 24 hours of AMI preparation,0.5 ml (2×107) stem cells was mainlined only;③Tong Guan capsule treatment group(n=10):After 24 hours of AMI preparation,0.5ml physiological saline solution was injected,2 Tong Guan capsules were given per day;④control group(n=10):0.5ml physiological saline solution was injected only. Capsules were given for every group during 6 weeks. After 6 weeks treatment, the following items were measured:the ultrasonic cardiogram index:LVDD,LVDS,LVEF,LVFS, heart weight, the number of cardiac muscle cells and veins around cardiac muscle infarct zone.2. The clinical study60 cases diagnosed AMI were randomly divided into the test group(30 cases)and the control group(30 cases). The control group was received appropriately conventional western medicine therapy, including treatment of thrombolysis, coronary artery expansion, anticoagulant, fat adjusting, speckle stabilization, nourishment sustainment and so on. Tong Guan capsule(3#, Tid)was received by the test group 3 months in foundation treatment. We observed the targets of ultrasonic cardiogram, such as LVEDVI,LVESVI,LVDD,LVDS,LVEF,LVSD,LVPWD and sf-36 list score on the first day and 90th day after surgery, assessed the heart function and life quality of AMI sufferer, and then compared between two groups.Results1. There are statistical difference(P<0.05) on the LVEd,LVEs,IVSd,LVEF,LVFS after six weeks between the control and test group at animal study. There are statistical difference (P<0.05) on the LVEs,IVSd,LVEF,LVFS between stem cells group, Tong Guan capsule treatment group and stem cells and Tong Guan capsule treatment group.2. There are statistical difference(P<0.05) on the heart weight and heart weight index by comparison of stem cells group, stem cells and Tong Guan capsule treatment group and control group. There are statistical difference(P<0.05) on the VEGF between all the test group and the control group. There are statistical difference(P<0.05) on the VEGF between all the stem cells group and the stem cells and Tong Guan capsule treatment group.3. There are no statistical difference(P>0.05) on the LVEDVI,LVESVI,LVDD, LVDS,IVSD,LVPWD,LVEF by comparison of the test group and the control group on the 1st day after surgery. There are statistical difference(P<0.05) on the LVEDVI,LVESVI,LVDD, LVDS,IVSD,LVPWD,LVEF by comparison of the test group and the control group on the 90th day after surgery.4. There is no statistical difference(P>0.05) on the every index by comparison of the test group and the control group on the 1st day after surgery. After 90days treatment, there is statistical difference(P<0.01or P<0.05) on the life quality index for each groups. The comparison between two groups shows that there is statistical difference(P<0.01) on body ache, spirit health, health change and general comment. And the index of test group is better than the control group. There is also statistical difference(P<0.05) on the physiological function and general health between two groups. But there is no statistical difference(P>0.05) on the energy, society function and feeling between two groups. Conclusion1. The Tong Guan capsule can effectively improve the echocardiography index (LVDd, LVDs, IVSd, LVFS, LVEF) in rats. The Tong Guan capsule can effectively increase the number of cardiac muscle cells and veins of the stem cell replanted of AMI rats.2. On the basis of conventional western medicine therapy, the Tong Guan capsule can effectively reduce the LVEDVI,LVESVI,LVDD,LVDS,IVSD,LVPWD, rise the LVEF, decrease heart expanding,cardiac muscle plumping and improve heart stretch function.3. It is available to improve the body ache, spirit health, health change, physiological function and general health so on for the Tong Guan capsule on the basis of conventional western medicine therapy. Which show that the Tong Guan capsule is able to improve the life quality of the sufferer.4. The Tong Guan capsule can improve the heart function of AMI sufferer, which is relevant to its protection for cardiac muscle and veins and its effect to anticoagulation.
