Clinical Observation On Treatmenting Patients With Stage 3 Chronic Kidney Disease According To The Academic Ideology Of Professor Zhang Qi

BackgroudChronic renal failure is the outcome of renal damage caused by various reasons and ingravescence. And the clinical symptoms are showed as the disorder of organism in excreting metabolic wastes and the disturbing of the water-electrolyte and sour-alkali balances of some endocrine active compounds and so on. In the category of TCM, Chronic renal failure belongs to the range of edema, dysuria, off grid and consumptive disease. The pathogenesis is very complicated. Stasis of evil domination accompanis deplation of righteousness. So, according to the academic ideology of Professor Zhang Qi-one of the National Masters of TCM, the rule of "baoyuan jiangzhuo" was uesd as the fundamental law of the treatment of Chronic Renal Failure. Professor Zhang Qi and his disciples applicated practice and have achieved good effect.ObjctivesTo observe the effect in treating CKD3 patients with the academic ideology of "baoyuyan jiangzhuo" established by Professor Zhang Qi. So as to improve our level of syndrome differentiation. And to examine our ability on mastering professor Zhang's academic ideology at the sametime.Methods30 patients with stage 3 CKD were brought into the study. All of them were inpatients or outpatiens of Guangdong Provincail Hospital of TCM. Everyone of them was given routine western therapies (include high-quality but low-level protein diet, correcting the disorder of water-electrolyte and acid-base balance, etc.), meanwhile, according to the difference on Syndrome of TCM. Acertain treatment among the eight methods in "Baoyuan Jiangzhuo" which established by Professor Zhang Qi was used. The course of treatment was 3 months and 2 courses were include in total. The changes of the items as clinical symptoms,Scr,eGFR,TC,TG,LDL,Hb,ALB,24-hour urine protein were observed before and after treatment.ResultAfter two courses of treatment,6 cases were significantly effective, 12 cases were effectual,11 caese were stable and 1 case was unfruitful with a total cure rate of 60%, stability rate of 36.7% and inefficient rate of 3.3%. The clinical symptoms and signs of 30 patients are obviously improved, aversion to cold,nausea and vomiting disappeared, basic elimination of edema, increased appetite, fatigue,shortness of breath,disinclination to talk,weak waist,abdominal distention,dizziness and other symptoms improved. Some patients had some new symtoms after treatment, such as after treatment 12 patients had varying degrees of fecal shapeless, 1-3times/day; fecal shapeless of 1 patient (lung and stomach yin deficiency and damp) was increased. A few patients had new symtoms which were nausea,dizziness and dry mouth, but the symtoms were mild.Compared with pre-treatment, at the point of 3 months and 6 months of treatment, the level of Scr decreased ((P<0.05) while the level of eGFR arised (P<0.05) Compared with pre-treatment, after 3 months of treatment, the level of LDL decreased (P<0.05), but the level of TC and TG were not changed (P>0.05). Compared with pre-treatment, after 6 months of treatment, the level of LDL,TC and TG were decreased. Compared with pre-treatment, after 3 months of treatment, Hb was increased (P<0.05). After 6 months of treatment, the level of Hb was rised, yet steady in the normal range; and the level of ALB was rised. Compared with pre-treatment, after 3 months of treatment, change of the quantity of 24-hour urine protein was not statistically significant (P>0.05); but after 2 courses of treatment, the quantity of 24-hour urine protein was decreased.ConclusionApplication of the academic ideology of "baoyuyan jiangzhuo" of Professor Zhang Qi on CKD3 patients can improve clinical symptoms, reduced serum creatinine, increased glomerular filtration rate, improve lipid metabolism, improve nutritional status, reduce urinary protein effectively retarded the progress of Chronic Renal Failure. There are some diffrernces in the effects between in our study with Professor Zhang Qi of Traditional Chinese Medicine Research Institute located in Heilongjiang Province. May prompt us to master the experience of Professor Zhang Qi there is still a gap. The clinical experience in the future the process of succession, we Should be more emphasis on learning-practice-and then learn-and then practice。practice to better grasp the name of clinical experience in Repeated study, and promote effective transmission.
