| Specialty:Nephrology of Traditional Chinese MedicineAuthor:Xu HailiTutor:Por.Liu XushengObjective:In our prospective self-controlled trial,Observation on the therapeutic effect of Traditional Chinese Medicine in treatment of the patients with chronic renal failure,which from Guangdong Hospital of Traidtional Chinese Medicine,according to the academic thought of "Baoyuan Jiangzhuo" of Professor Zhang Qi,and discuss on how to improve the level of treatmentwith Traidtional Chinese Medicine.Method:We treated the incorporating patients with chronic renal failure by Chinese medicine on syndrome differentiation(Eight metheods in Baoyuan Jiangzhuo by Professor Zhang Qi) and western medicine:High-quality low protein diet,correction water and electrolyte disturbance and acid-base imbalance by prospective self-controlled method.The test of hemoglobin, serum creatinine,creatinine clearance rate,blood lipids and serum albumin had taken once two weeks.A course of treatment lasted three months,and every patient had two courses.Then a efficacy evaluation of treatment should been taken.and we should measure the patient's quality of life by the World Health Organization.quality of life measured BREF(WHOQOL=BREF).Asking for the help of covariance software of the SPSS13.0,we could to analyse the relations of the clinical symptoms and laboratory data of the renal failure patients before and after the treatment,to explore the clinical significance of the treatment on the chronic renal failure through the academic thought of "Baoyuan Jiangzhuo" of Professor Zhang Qi.Results:The preliminary findings showed that:In our study,60 patients were divided into heat resistance Turbidimetry License(covered 8 cases,13.3%), heat toxic and blood stagnation(11 cases,18.4%),heat Yun node in the coke card(covered 6 cases,10%),Qi-Blood deficiency of the spleen and the tomach(8 cases,13.3%),pleen-kidney deficiency of Yin-yang(7cases, 11.7%),hypofunction of Spleen-Kidney and heat-toxicstagnation and bood stasis(10 cases,16.7%),according to traditional Chinese medicine of syndromes before treatment.heat-toxic stagnation and bood stasis(7cases, 11.7%),Qi-Blood deficiency of the spleen and the stomach(9cases,15%), Spleen-kidney deficiency of Yin-yang(13 cases,21.7%),hypofunction of Spleen kidney and heat toxic stagnation and bood stasis(16cases,26.7%) were differentiated post-treatment.By TCM frequency comparison,the resuts showed:heat-toxic stagnation and bood stasis(104 cases,14.5%),Qi-Blood deficiency of thespleen and the stomach(99 cases,13.8%),Spleen-kidney deficiency of Yin-yang(120 cases,16.6%),hypofunction of Spleen-kidney and heat-toxic stagnation and bood stasis(152cases,21.1%)were differentiated.Correlation analysis results showed that:by the T test,the patient's level of serum creatinine at first visit and 3 months after the comparison, there was no significant difference(P＞0.05),the first visit and six months later,3 months and 6 months after,The difference was statistically significant (P＜0.05),the level of serum creatinine after treatment decreased than the first visit;the creatinine clearance rate,the differences were statistically significant in the three treatment comparison(P＜0.05),the serum endogenous creatinine clearance rate showed an upward trend;the hemoglobin and the cholesterolthe differences were statistically significant in the three treatment comparison(P＜0.05),the hemoglobin rised,and the cholesterol lowered;the comparison of the triglyceride connection between the first visit and 3 months later was different(P＞0.05),the first visit and six months later, three months later and six months later,the difference was statistically significant(P＜0.05),the triglyceride lowered;the comparison of the albumin between the first visit and six months later was different(P＜0.05),the albumin rised,the comparison of the albumin between the first visit and three months later,3 months and 6 months later was not different(P＞0.05).According to the 2002 clinical study of new Chinese medicine as the guiding principles of chronic renal failure clinical efficacy criteria,11 cases were cured(18.3%),37 cases were effective(61.6%),14 cases were stable(23.3%), nine cases were invalid(15%),The total effective rate was 61.6%.According to the World Heath Organization Quality of Life BREF,comparing changes of life quality pretherapy and post-treatment indicating:there are significantly different in the feeling and ablility in working,life satisfaction,quality of life etal.Conclusion:Application of Baoyuan jiangzhuo thought of professor Zhang Qi in treatment of patients with chronic renal failure based on syndrome differentiation,it was helpful in delaying progress of renal failure,improving the clinical symptoms,lowing serum creatinine,enhancing endogenous creatinine clearance rate,improving lipid metabolism disorder,improving the nutritional status and quality of life of patients. |
