Objective:To investigate the pathological changes and Local IL-2,IL-6,TNF-α,TGF-β1,CTGF,PDGF content of rat inflammatory oviduct by inoculating Salvia miltiorrhiza injection on it, study the effect and preliminary mechanism of Salvia miltiorrhiza injection on the inflammatory fallopian tube。Methods:Ninety female SD rats of 8-10 weeks old, weighting 200-240g, were randomized into three groups unilateral model group 48, bilateral model group 36, normal group 6, Which were inoculated with mixed strains on the tube to establish the model. After general anesthesia and sterilization, open the abdomen, find the fallopian tube, puncture near the uterine horn, injecting in the direction of fallopian tube-ovarian with the mixed bacterial solution (E. coli, Staphylococcus aureus, Streptococcus dubbed by 2:1:1 ratio of about 3 X 109/ml concentration of the mixture), unilateral model group were injected with 0.1ml on left side; bilateral model group were injected with 0.1 ml on each side, normal group were injected by saline with 0. 1ml on left side.20 days after modeling, pathological changes of fallopian tube show inflammation, the survival rats of both model groups were randomly separately divided into 5 groups:low, medium and high dose of Salvia miltiorrhiza(Sm) group, untreated group and Western medicine(Wm) group. unilateral group:the total is 45, each group 9;bilateral group:the total is 26, each treated group 6, the Blank model (Bm) group 2. Inject drugs or saline from the uterine cavity to the fallopian tube, Sm groups with Salvia miltiorrhiza injection, Wm group with the traditional anti-inflammatory drugs (gentamicin, dexamethasone, a-chymotrypsin), Bm and normal groups with saline, once a week, totally 3 times. observe the pathological changes and Local IL-2,IL-6,TNF-α,TGF-β1,CTGF,PDGF content of rat inflammatory oviduct. Collect the clinical cases diagnosed as salpingitis infertility and accepted interventional recanalization successfully, which were randomly divided into SM group and western medicine group, flow liquid to the uterine cavity once after the operation, and repeat it on 3-7 days after the next period, totally 2 months, the Sm treatment group with diluted Salvia miltiorrhiza injection and with the Oral and externally used Chinese Medicine Virtual Screening Based on Pharmacophores, western medicine group with mixed dilution (gentamicin, dexamethasone,α-chymotrypsin). Followed up for 12-month, nopregnant people were reviewed tubal patency by hysterosalpingography, then we compared the pregnancy rate, patency rate and occlusion rate of the two groups。Results:1. The unaided eye:the blank model group can be seen pelvic adhesions in varying degrees, fallopian tubes become thinner and appear rough, pale color, adhese with more or less surrounding tissue, flexibility is poor, fragile and vulnerable, and empyema was found in individual tube;each treatment group were improved in varying degrees than the blank model group, especially the Sm group, near normal performance. Compared with unilateral model group, Bilateral model group can been seen obviously a more serious degree of pelvic adhesions, fallopian tubes and surrounding tissues often wrapped into a group, tubal look more pale, brittle and fragile.2. Light microscope:the blank model group:the oviduct wall structure are unclear, lumen are nearly patent, serosal layer are infiltrated by many lymphocytes and plasma cells, individual show small abscess, small vascular congestion, muscle layer can see marked proliferation of fibrous tissue infiltration. The treatment groups have different degrees of improvement compared with the black model group, there is no obvious difference between bilateral and unilateral group model.3. Cytokines results:the blank model group IL-2, IL-6, TNF-α, TGF-β1, CTGF, PDGF content of oviduct were significantly higher, by statistically, IL-2, IL-6, TNF-α, CTGF, PDGF (P<0.05), TGF-β1 (P<0.01); All the treatment groups'content of the above-mentioned cytokines were lower than the blank model group, by statistically, there were no significant differences between any two groups(P>0.05); IL-2:Sm medium, high dose and Wm group in unilateral model group were significantly decreased inlevers(P<0.05), other treatment groups show no significant difference(P>0.05); IL-6:H-Sm group in unilateral model group, M-Sm, H-Sm andWm group in bilateral model group were significantly lower (P<0.05), other groups show no significant difference(P>0.05); TNF-a:M-Sm, H-Sm and Wm group in unilateral model group, Wm group in bilateral model group were significantly decreased (P<0.05), each Sm groups in bilateral model group were reduced very significantly (P<0.01); TGF-β:M-Sm,H-Sm andWm group in unilateral model group were very significantly decreased(P<0.01), L-Sm group in unilateral model group and all the bilateral model groups were significantly reduced (P<0.05); CTGF:M-Sm, H-Sm and Wm group in unilateral model group were significantly decreased (P<0.05), other treatment groups show no significant difference (P>0.05); PDGF:all the treatement groups were decreased, but there was no significant difference (P>0.05). Clinic research:Sm injection, etc. Traditional Chinese Medicine are more effective than wester medicine or salpingitis infertility patients after interventional recanalization, by statistically, there was significant difference (P<0.05).Conclusions:1. the Streptococcus, Staphylococcus, E. coli bacteria mixed can make rat oviduct infiltrated by inflammatory cells and collagen proliferation, IL-2, IL-6, TNF-α, TGF-β1, CTGF, PDGF in the local organizations was significantly increased, a chronic salpingitis and fibrosis rat model has been established.2. Local administration of Sm injection inhibited rat oviduct inflammation, reducing the IL-2, IL-6, TNF-αcontent of local organizations is the possible mechanism.3. Local administration of Sm injection inhibited rat oviduct fibrosis, reducing the TGF-β1, CTGF content of local organizations is the possible mechanism.4. Sm injection can keep the tubal patency and increase pregnancy rate for salpingitis infertility patients after interventional recanalization.
