IntroductionThe management of breast carcinoma has developed from simple surgery to a systemic comprehensive therapy, in which neoadjuvant chemotherapy(NCT) takes a very important part.NCT is good for the breast carcinoma with the recent response rate and it offers the credible evidence to susceptibility test of chemotherapeutic agent in vivo. NCT also can increase the opportunity of the breast-conserving surgery and improve the survival and prediction of prognosis. But the key question is how to choose a new adjuvant chemotherapy for individual programs to improve the pathological complete remission rate, monitoring of therapeutic efficacyNCT efficacy evaluation methods include two ways that is pathological evaluation and clinical evaluation. Pathological evaluation was executed after surgery so it can not provide guidance for clinical treatment.The clinical evaluation includes some methods that is palpation, color Doppler ultrasonography and mammography etc.Our study summed up the sonographical characters in relationship with the effect of neoadjuvant chemotherapy.The result was compared with that of pathological examination.The purpose is to investigate the value of ultrasonography in evaluating effect of neoadjuvant chemotherapy to breast carcinoma.Materials and methods1.Objectives:43 patients with breast carcinoma were undergone operation at breast surgical department in Liaoning tumor hospital.All the patients were given CEF(CTX+EPI+5-Fu). The total lesions were 43.2.Methods:All the US was performed using a probe with frequency 7-12MHz(LOGIQ9 US system, or LOGIQ7 US system, made by GE company USA). Operator should scan breast and axilla and use a clockwise, sequential, overlapping radial approach.When detect the lesions the operator records the lesion number, size, shape, intensity of internal echoes, microcalcification. All the lesions were undergone CDFI. The blood flow were divided into 0-â…¢class according to Alder semiquantitative method and record RI values.Clinical Evaluation is according to RECIST standard, pathological evaluation is according to the classification method of Miller&Payne.3.Statistical analysis:Chi-square test and Fisher's exact test is applied for comparison of the rate.T-test is applied for the values of RI.The above statistical analysis were conducted with the statistical software package SPSS for Window, version 10.0.ResultsUltrasonography detected 43 lesions of all 43 breast lesions. According to the length of breast carcinoma lesion, the accuracy, sensitivity for palpation in evaluating effect of neoadjuvant chemotherapy were 34.89%,59.09%. According to the size of breast carcinoma lesion, the accuracy, sensitivity for US in evaluating effect of neoadjuvant chemotherapy were 72.09%,86.36%. In partial remission group, there were significant differences in the blood mode and RI value of the breast tumors between before and after the chemotherapy(P<0.05).In unchanged group, there was no difference in the blood mode and RI value of the breast tumors between before and after the chemotherapy(P>0.05). Conclusion1.The chages in the size of breat carcinoma lesions can be usd to evaluating the effect of neoadjuvant chemotherapy.2.The chages in the blood mode and RI value of the breast carcinoma lesions can be usd to evaluating the effect of neoadjuvant chemotherapy.3.It was useful to appraise the effect of neoadjuvant chemotherapy to breast carcinoma by Color Doppler Ultrasonography.
