Objective:Tooth defect is one of the common stomatology diseases. Restorative treatment is preferred for large range tooth defect, as satisfied result can not be obtained by simple filling treatment. United States has been using the metal ceramic restorations clinically since 1950. With the development of dental materials, people nowadays hold higher standards of appearance. Therefore, all ceramic crown are gradually taking the place of the metal ceramic in clinical restorations due to its natural color, fine gingival margin coloring, appropriate transparency and good biocompatibility.IPS e.max Press system is Ivoclar Vivadent's new generation all ceramic system after the IPS Empress 2 system. IPS e.max Press is lithium-disilicate reinforced ceramic, which has a bending strength of 400 MPa and various levels of transparencies. Comparing with IPS Empress 2 system, IPS e.max Press System has a higher strength.IPS e.max Press has three transparencies for different individuals. The restorative effects of anterior tooth and premolars defect after IPS e.max Press MT, HT ceramic crown restorations, as well as posterior tooth defect after IPS e.max Press LT ceramic crown restorations, are studied and evaluated clinically.Method:35 patients with 56 anterior tooth and premolars defect restorations with IPS e.max Press MT, HT ceramic crown, as well as 63 patients with 98 posterior tooth defect restorations with IPS e.max Press LT ceramic crown are selected are selected, the restorative effects are evaluated on color, shape, margin coloring, margin sealing and second caries after 3-18 months according to the modified criteria of APHA (American Public Health Association). The satisfactory degrees of restoration's color, shape, function and comfort are investigated at the re-examination. Moreover, the rates of porcelain fracture of 164 restorations are also evaluated.Results:1. Anterior tooth and premolars defect after IPS e.max Press MT, HT ceramic crown restorations:(1) Color:after 3,12months,98.21% scored A,1.79% scored B,0% scored C; after 18months,96.43% scored A,3.57% scored B,0% scored C;(2) Margin coloring:after 3,12,18 months,100% scored A,0% scored B,0% scored C;(3) Second caries:after 3,12,18 months,100% scored A,0% scored B;(4) Shape:after 3,12,18 months,100% scored A,0% scored B,0% scored C;(5) Margin sealing:after 3,12,18 months,98.21% scored A,1.79% scored B,0% scored C;(6) Patient's satisfactory degree:for color and shape,94.29% satisfied,5.71% basically satisfied and 0% dissatisfied; for function and comfort,97.14% satisfied, 2.86% basically satisfied and 0% dissatisfied;(7) There is one case of porcelain fracture occurred, occurrence rate 1.79%. 2. Posterior tooth defect restorations with IPS e.max Press LT ceramic crown:(1) Color:after 3,12,18 months,93.88% scored A,6.12% scored B,0% scored C;(2) Margin coloring:after 3,12,18 months,100% scored A,0% scored B,0% scored C;(3) Second caries:after 3,12,18 months,100% scored A,0% scored B;(4) Shape:after 3,12,18 months,100% scored A,0% scored B,0% scored C;(5) Margin sealing:after 3,12,18 months,97.96% scored A,2.04% scored B,0% scored C;(6) Patient's satisfactory degree:for color and shape,95.92% satisfied,4.08% basically satisfied and 0% dissatisfied; for function and comfort,97.96% satisfied, 2.04% basically satisfied and 0% dissatisfied;(7) There is no porcelain fracture occurred.Conclusions:IPS e.max Press is a good dental material which has a satisfied clinical restoration effect.
