Objective: To assess the effect of microsurgical treatment for intracranial aneurysm and analysis on the factors effecting its, therapeutic thus to improve the prognosis quality of life of the patients and share in our experience .Methods: a retrospective analysis of intracranial aneurysms cases which have done CTA inspect of 143 clinical data during Jane 2007 to Octo 2009 at jilin university first hospital dept two.According to the location, form and size of aneurysm, and combined with Hunt - Hess grade and CT Fisher grade of patients on admission to select patients who fit operation.Select a concrete operative approach, according to the different patients.Strengthen the disposition of perioperatie. According to the GOS prognosis grading of patients on discharge to analysis the effect of operation. singular factor analysis and multiple factor analysis was used by SPSS l3.0 satistics software to analysis the related affecting prognosis factors such as age, sex, Hunt-Hess Classification of SAH, location and size of aneurysms, time of operation, Intraoperative rebleeding , CT Fisher's grade for SAH,complication after operations.Results: All of the patients have done CTA inspect, and has proved as aneurysm. All of 143 patients with 171 aneurysms were surgical treatment, 117 cases of all had been surgically clipping, both clipping and wrapped in 5cases,2 cases were isolated, wrapped in 16 cases, pseudoaneurysm resection in 1 case, According to 143 patients discharged GOS rating,we regard grade 4 and 5 as good outcomes. There are 104 cases, account for 72.73 %. Grade 3 and the follow were regared as poor prognosis. In our group,there are 39 cases ,account for 27.27%. Aneurysm rebleeding in six cases.One patients died for multiple organ dysfunction syndrome (MODS) after operation. The prognosis of patients was obviously correlated with Hunt-Hess grade (P=0.000), CT Fisher grade (P=0.000) and complication after operations(P=0.000).Conclusions:1) Microscopic neurosurgry operation was an effective Method to treat intracranial aneurysms.2) Therapeutic of microscopic neurosurgry operation depend on the Hunt-Hess grade, CT Fisher grade before operations, and whether the patients combined with complications. Hunt-Hess grade and complication after operations is the most influencing factors. If want to ascertain the effect of each factor on prognosis of AN patients and get exact mathematical model, we need do prospective analysis, add amount of samples,and train special messenger to collect data.
