OBJICT: To discuss the clinical diagnosis and surgical intervention of mutational falsettos.METHODS: Patients with mutational falsettos in the Department of Otolaryngology-Head & Neck Surgery, First Affiliated Hospital, Guangxi Medical University, from January 2000 to December 2010 were selected. Clinical diagnosis, operations, curative effect and prognosis of these cases were analyzed retrospectively and available literatures on this entity were reviewed.RESULTS: 18 cases met the clinical diagnosis standard between Jan 2000 and Dec 2010. The subjects had a mean age of 21.9 years. With the characteristics of high-pitch all patients onset of illness in puberty, companion have hoarseness or easy to fatigue. 3 patients were not needed operations because they were too young, 1 patient disagreed with operation and the rest were selected laryngeal framework surgery. Before and after operation subjective auditions were conducted by otolaryngologists. Except 1 patient's pitch, pitch of 13 patients lowered to normal level of adult man after operation. There were no early or late postoperative complications. 5 cases were followed-up 1 year to 8 years and no cases recurred.CONCLUTIONS: Laryngeal framework surgery can be performed as a possible treatment for lowering the vocal pitch in cases of mutational falsetto after failure of conservative therapy. Laryngeal framework surgery include type III thyroplasty , modified relaxation Isshiki(type III)thyroplasty , window relaxation thyroplasty.
