Objective:Displaced neck of femur fracture is a common clinical problem among the elderly population. Our aim is to review previously published randomized controlled trials to establish if total hip arthroplasty(THA)or hemiarthroplasty will offer a superior clinical outcome in this group of patients on rates of mortality, reoperation, pain, function, operating time, and general complications in patients with a displaced femoral neck fracture.Methods:We searched computerized databases for randomized clinical trials published between 1960 and 2010,and we identified additional studies through hand searches of major orthopaedic journals, bibliographies of major orthopaedic textbooks, and personal files. Of 103 citations initially identified,8 met all eligibility criteria. Two investigators independently graded study quality and abstracted relevant data, including information on revision and mortality rates.Results:Two trials, which included a total of 178 patients, provided the Duration of surgery, and the overall z=5.18,p<0.00001, which means that THA need more time to accomplish the operation. We found no significant differences between the two methods of treatment for general complications, dislocation, reoperation, and mortality (p>0.05). The harris scores of the THA group in 1 and 4 years after operation is significantly higher than that of HA's(p<0.05).Conclusions:In comparison with hemiarthroplasy, total hip arthroplasty for the treatment of a displaced femoral neck fracture have no advantage in reducing the reoperation, mortality, dislocation and complication rates, but it can significantly eliminate pain and improve the funtion of body at the cost of longer operative time.
