The Clinical Value Of Detection Of Serum CEA,CYFRA21-1,NSE For Diagnosis In Patients With Lung Cancer

Objection:To study the clinical significance of detection of serum CEA (carcino-embryonic antigen),CYFRA21-1(cytokeratin 19 fragment),NSE (neuronspecific enolase) for diagnosis in patients with lung cancer by ECLIA (electrochemiluminescence immuno-assay).Material and methods1.General condition of the patients:140 cases with newly definite diagnosed of lung cancer admitted into Shandong Provincial Hospital from October 2007 to September 2008 were enrolled into our study,including 89 males and 51 females,average age 59.64 years(ranged 23-89 years);122 cases of non-small cell carcinoma,18 cases of small cell lung cancer.All cases were confirmed by pathology or cytology.The patients was selected at the same time meeting the following criteria:1) Normothermia;2) Routine blood test revealed no abnormalities;3) No yellow sputum,or other sings of infection;4) Not simultaneously with benign lung diseases;5) exclusion of other malignancies; 6) Without liver nor kidney diseases.189 cases of benign lung disease that were Shandong Provincial Hospital inpatient at the same period were enrolled into our study,including 124 males and 65 females,average age 55.27 years(ranged 14-83 years); including cases of COPD,bronchiectasia,pneumonia,pulmonary tuberculosis,tuberculous pleurisy et al,and no other malignancies or liver or kidney diseases.2.Reagents and equipment:ECLI analyzer(Elecsys 1010) and detection kits for CEA,CYFRA21-1 and NSE were provided by Roche Company. 3.Collection of specimens:Fasting peripheral venous blood 3ml were taken before diagnosis for all cases,which were natural solidification at room temperature,separation serum and detected at once.4.Assay method:Serum CEA,CYFRA21-1 and NSE were detected by ECLIA, strictly following the instruction manual of the analyzer and detection kits.Any outcome exceeding 10ng/ml,3.3ng/ml and 16.3ng/ml for CEA,CYFRA 21-1 and NSE respectively was positive.5.Statistical method:Levels of serum CEA,CYFRA21-1,NSE were expressed by ((?)±s),the rate difference of two groups were compared by chi square test,and all by SPSS 13.0(for windows). Conclusion1.Levels of serum CEA,CYFRA21-1,NSE in lung cancer cases were significantly higher than those in benign lung diseases cases.2.The sensitivity of combinated detection of serum of CEA,CYFRA21-1,NSE in the diagnosis of lung cancer was much higher than those detected separately.3.The levels and the positive rates of serum CEA and CYFRA21-1 in NSCLC were much higher than those in SCLC,while the levels and positive rate of NSE in SCLC was significantly higher than that in NSCLC.The positive rate of combinated detection of serum of CEA,CYFRA21-1 and NSE significantly rised in both SCLC and NSCLC. 4.Pay attention to those with high levels of serum CEA,CYFRA21-1,NSE with benign lung diseases,especially the patients with COPD.
