| Objective To evaluate the clinical application value of 1H-MRS in brain tumors by studying the feature of 1H-MRS in brain tumors.Materials and methods 30 patients of intracranial neoplasm were studied(17 male,13 female, mean age 51 years), all of them were approved by pathology or clinician, consisting of 10 cases of Low-grade gliomas(grade I and grade II),8 cases of High-grade gliomas (gradeIII and gradeIV), 5 metastasis, 5 meningiomas, 2 cerebral radiation necrosis .The 30 patients underwent 1H-MRS and all 1H-MRS were gained on plain MRI scan. Muitiple voxel Proton magnetic resonance spectroscopy was performed on a 3.0T MR scanner.The variety of metabolite peaks were detected in different areas. By exploring the feature and clinical application of 1H-MRS in brain tumors,we can evaluate the clinical worthiness of 1H-MRS in preoperative diagnosis, differentiation, pathologic grade, and foretell the patient's prognosis of brain tumors .Results(1)~1H-MRS of neurogliocytoma:the value of Cho/Cr and Cho/NAA differ between high-grade neurogliocytomas and low-grade neurogliocytomas (p<0.05) . (2)The discrepancy of Cho/Cr value in different brain tumors:the value of Cho/Cr differ between meningeomas and other brain tumors(p<0.05). The Cho/Cr value from neurogliocytomas were similar to that from metastatic tumors in tumor parenchyma.(3)The discrepancy of Cho/NAA value in different brain tumors:the value of Cho/NAA differ between low-grade neurogliocytomas and other brain tumors(p<0.05).But the Cho/NAA value from high-grade neurogliocytomas were similar to that from metastatic tumors in tumor parenchyma.(4)The value of Cho/Cr differ between low-grade neurogliocytomas and high-grade neurogliocytomas,high-grade neurogliocytomas and metastatic tumors in peritumoral edema (p<0.05) .(5)1H-MRS of cerebral radiation necrosis:there were high Lip and Lac,slightly higher Cho,no or low NAA and Cr in cerebral radiation necrosis.Conclusion1. ~1H-MRS is the effective method for diagnosing brain tumors.2.The combination of 1H-MRS and MRI can improve preoperative diagnostic accuracy of brain tumors.3. ~1H-MRS has made it possible to evaluate the grade of gliomas,obtain metabolic information of gliomas ,reflect the potential about development of tumor and provide aids for treatment programmes.4. ~1H-MRS has some aids for the differentiation of brain radiation necrosis and tumor recurrence. |
